We saw Dr. Carey yesterday

Well we met with dr. Carey in London yesterday. He said -'s

left fooot has definetly relapsed. He said he has a very " lazy "

muscle running up the inside of his leg whick is making it very

difficult to hold the correction. He has prescribed an AFO for him

to wear for teh summer and he will see him in Spetember for serial

casting... again! He said he will most likely need the tendon

transfer done when he is 4 or 5.

So my questions are... what do you think?

I have called to get an AFO for his left leg and we wouldn't beable

to get it before the middle of August! Thst seems a bit much. Does

the AFO need to be molded to his foot or do we just get his size?

Also, what do you think of him waiting to do the serial casting until

September?

But as always, Dr. Carey was just amazing with teh way he treated my

little guy. He even checked 's stuffed Monkey's feet to see how

they were. He is great!

I think this sounds pretty reasonable for conservative treatment,

although I do have some questions for you. You didn't mention if

- is still wearing the FAB at night. Did Dr. Carey talk about

increasing his hours in the brace at all? Is the AFO just for during

the day? Is he just waiting until September because he doesn't want

- to miss out on summer activities? I know that Dr. Ponseti

*usually* recommends not waiting to treat a relapse -- from the global

help-booklet: " Do not ignore relapses! At the first sign of relapse,

consider reapplying one to three casts to stretch the foot out and

regain correction. " But I have also heard of him waiting a bit so it

would probably have to be on a case by case basis for this decision.

There are different types of AFO's - some are stock sizes and some are

custom molded. But what is the point of getting it if they are not

going to get it to you until August?, I would say that is unacceptable

and Dr. Carey should be able to do something about this if you let him

know that you are experiencing this problem.

The main thing is that, while casting an active toddler (I forget how

old - is) may seem daunting, it can be the only treatment that

is needed to get that foot back to (and staying) perfect. And then

it's kind of a wait and see on the tendon transfer.

Let us know more and we can give you a little more insight.
