At least she doesn't have to be in a cast for a terribly long time...although I know it probably feels like it will be forever!! She is probably more uncomfortable at night simply because she is tired out from the daytime. I know I was that way after my c-sections...I was ok during the day but was much more painful in the eveningtimes when all of my activities during the day kind of caught up with me. Jake was the same way when he was in casts (he's had surgery on his clubfoot twice, and broke his leg (femur fx) a year ago. You might try giving her the motrin on a schedule (around the clock), making sure she gets it maybe an hour or so before bedtime; then it will have time to kick in before she goes to bed. Also, elevating the cast leg with a pillow or even a folded blanket may help. When people lay flat, it's much easier for swelling to build up in the cast leg, which is uncomfortable. Elevating it will help the blood/fluids to return to the heart more quickly which decreases swelling and pain. Her wakefulness at night may also be due to her decreased movement- I'm sure it's a big adjustment for her to have to move that heavy cast when she wants to turn in bed.
