What is this about doing the patient contact list from

an e-mail form? I feel like the rules keep changing. I

originally wrote my 5-10 page " Patient Letter " only to

go back and re-write it so that it would be 10-11

pages. Like you, I began this journey back in August.

I have contacted more than 10 patients by phone and

asked my own questions. I'm waiting on results from

pap and then I will be ready to send my package. I

hope I don't have to redo the patient contacts. If any

one has some insight please speak up. I want to cross

all the t's and dot all the I's.

Ok I didnt do my contacts online either...I thought we had a choice and

could do either or? Im I wrong?

--- Maragret and I didn't do our lists on line either.

Barbara did send me and email this week and said I

needed to " summarize " my contacts since all I did was

send the copies of the email I got on each one. But

that was before they said anything about doing it " on

line " I hope they don't come back and say I have to do

it that way now, I've been waiting all week for my

answer so I could schedule my appt.

I'll keep my fingers crossedXXXXXXXXX!!!!

I already have all of my contacts printed out!! I used the same format that

the online form does.. do I still have to fill out the online form

instead?????

Hey I am in the same boat. When I mailed in my packet this past wed. I

believe it was up to us whether we wanted to do it online or by mail with

our packets. I also had my own set of questions that I asked so I hope

that is ok. I havent received anything my email yet....I would love to

hear what someone else has to say also.

Robin,

I sent in my packet about 1 1/2 weeks ago, that was a day after he

posted the online form.

I went ahead and done the online form tooo,,.

So, there would be no questions..

P.S.

I used different contacts for the online form..

I had contacted so many, so I put them to use...

in Ga
