Gloria,

I am trying to feel out the on line contact form just

in case that is the hold up on my packet. You had

agree to be a contact for my and all I need is your

current weight for Dr. R's form. He is asking for

First and Last Name, email address, date of MGB,

weight to start and current weight.

ANYONE WHO WOULD BE WILLING TO BE A CONTACT FOR ME,

PLEASE E-MAIL ME OR POST THE INFO ON E-GROUPS. I NEED

ABOUT 3 MORE!! I didn't keep a copy of what I sent

with my packet so I have had to do some back tracking.

Please Help!
