Cystic Fibrosis Research, Inc. Awards

The Cystic Fibrosis Research, Inc. awards will be given out this year at the

CFRI Annual Educational Conference held at the Sofitel Hotel San Francisco

Bay on August 7, 2004. This year CFRI will again honor two individuals who

have demonstrated excellent work or volunteerism in the field of CF. The

Dave Stuckert Award will be given to an outstanding volunteer, and a CF

Professional Award will be given to someone working in the field. A special

panel, consisting of past CFRI award recipients, has been selected to rank

and select the recipient according to their level of contributions.

The criteria for nominations are:

¨ service above and beyond the call of duty to the CF community at

large

¨ excellence in respective field

¨ outstanding contribution to the CF community

Guidelines for nominations:

¨ Anyone can submit 2 names as prospective nominees.

¨ Give a description of 100 words or less on each nominee.

¨ Send nominations to CFRI via mail, e-mail (progmgr@...), or fax

() by May 7, 2004.

¨ No nominations will be taken over the phone.

Judging Procedures:

1. After the cut-off date of May 7, 2004, the CFRI conference committee

will narrow the nominations down to the 10 finalists…5 for the volunteer

category and 5 for the professional category.

2. The judging panel will be sent (via e-mail, fax, or U.S. mail) the

descriptions for each of the finalists. Each panelist will rank the

finalists on a scale of 1-10.

3. When all rankings have been turned in, the scores will be tallied.

The nomination with the highest score in each category will be given the

award.

4. In the case of a tie, the 2 finalists will be sent back to the panel

for reevaluation.

Please submit your nominations with description of nominee to my attention

before May 7, 2004. If there are any questions, please do not hesitate to
