I just read in the Time magazine article about weight loss surgery, the reason we dont regain it is because our surgery (MGB) offers both restrictive eating and less absorption of calories. And about the food backing up, I asked my PCP this same question and he said that sometimes the food does back up into that old intestine but the body naturally moves it in one direction only. The muscles of the intestines are constantly working the food OUT of your body not back up into it. Therefore, what little food that may back up is naturally pushed back out. Hope that helps.
