Questions about nebulizer treatment

I posted an introduction couple of weeks ago. My daughter Kaley is

8wks old and has CF. She just came home from a 5 day stay at the

hospital for RSV. They send us home w/ Albuterol and Pulmozyne,

among other things. The directions for Pulmozyne are once a day,

and the albuterol is " as needed " . In the hospital when the

respiratory therapists did her treatment, they always did the

albuterol, then the pulmozyne. My question is do I need to give her

the albuterol before the pulmozyne, or can I give her the pulmozyne

by itself?

And I have a question about enzymes. she was started on 1 capsule

of ultrase at 1 week old, and has been having that with each feeding

since then. I would imagine that as she gets bigger she'll need

more, but hasn't yet. Are there test that can be done to see if she

actually needs the enzymes? She started out at 50% for height and

weight, and is now at the 75th% for height and weight. I just

assumed since she's growing so quickly at this age that we would be

increasing her enzymes frequently, but since she hasn't shown signs

of needing more, I wonder if she needs them at all. Any thoughts?

Hi Jenn,

I was told by our clinic that we should always do the albuterol before the

pulmozyme. The Albuterol will open the airways up and the pulmozyme will make

the mucus thinner and easier to move. It makes sense to have the airways as

open as possible before you do the pulmozyme so it can 'get down in there " (as I

was told) If you are unsure of anything regarding the treatments you should

call clinic though just to make sure you are following the directions that they

want you to follow. One of the nurses will probably get back to you. That is

what they are there for.

There are tests to see if she actually needs enzymes. I can't remember the

names......one is a stool test to measure how much fat is in there and the other

tests the stool for........... something that I can't come up with right now but

it stars with " E " (Elastese? it reminds me of elastic every time I read/hear

it....) Anyway, it's great that your daughter is in the 75%.

Hi Jenn,

I can't give you any info on the neb treatments since we have not had to

start those yet. I can tell you that my daughter had a test (stool) to

see whether she was pancreatic insufficient or not. She has been on

enzymes since she was born. Like your daughter she was on 1 ultrase

with each feeding. She was doing so well with weight and height that I

asked them to do the test. I wanted to be sure she needed them. Sure

enough the test showed she needs them. Anyway she did not need to

increase her enzymes until we started solids at about 6 months. I think

it is more related to what they eat than their size. My daughter is 14

months now and she is on 2 ultrase with each meal (including when I

nurse her). She is also 22lbs... I do have to add that she takes

Previcid for reflux and that does help the enzymes work better. When

I've tried taking her off the Previcid she has signs of needing her

enzymes increased. Hope this helps.
