I have read on here that some people are on 75, 100, 150. How long does it take to loose the effectiveness of a dose and need to go to a higher dose. Anyse, When I started on 25 mgs. duragesic in February, 2003, I remained on that dose for approximately six months. After my diabetic ketoacidosis attack and a resumption of pain from the pseudocysts in June of 2003, I increased to 50 mgs. and remained on that dose for another 6 months until I was hospitalized with an acute pseudocyst inflamation attack a year ago and a month later had to have an increase. For the past year I've been on 75 mgs, initially on a 72 hour rotation. My PM doctor just changed my rotation to every 48 hours two months ago, instead of increasing the dosage again. This has worked out very well for me. I hope this helps. You must remember that we are all different, though, and some people's pain increases at a faster rate than others.
