She is a terrible nail biter!! I can't understand how he thinks they look like they are clubbing. She has a little damage to her lungs about the size of the end of an eraser on the end of a pencil. But then again, there are different types of finger shapes: conical, square, and some people have spatulate finger tips, which sort of resemble the first stages of clubbed fingers.
