Joze,

I am currently being treated by meds for SOD. I am type III and my

doc at Mayo thinks the surgery is too risky for me and being that my

labs (liver enzymes etc) have never been elevated, I am not exactly

an eligible candidate for the sphincter surgery. I started on

Procardia 30 mg daily and now I am up to 60 mg daily. I have only

been on the meds for a couple of months so I wouldn't be able to

tell you how it works in the long run. I understand that it takes

several months to see results. But I can tell you that my pain has

definately decreased in intensity. I do still have pain daily as I

always have but the intensity and duration have decreased.

It helps, dear Zoie, thank you very much indeed.

It is very important that you told me the name and

dose of the drug! That's it! The fact that you have

got the med prescribed from a Mayo doctor is important

as well. The Mayo Clinic is still a symbol of the best

medicine in the world...

I rushed to the websites referring to Procardia. May I

ask you: how about your blood pressure? It seems that

Procardia (nifedipine) lowers the BP?! I have a normal

BP by now.

Treatment of Sphincter of Oddi Dysfunction

Antispasm medicines are available, but are not very effective. A decision

has to be made whether to cut the sphincter (sphincterotomy), during ERCP,

or at surgery. When sphincter of Oddi manometry has confirmed that the

pressures are high, sphincterotomy gives complete relief in 70-80% of

patients; unfortunately, sphincterotomy also carries a significant risk of

complications in this context, particularly the provocation of pancreatitis

, and the possibility of perforation. Thus, this condition should be

approached and managed with considerable care. Patients may warrant referral

to specialist centers.

Sphincterotomy procedures can sometimes scar after months or years, causing

papillary stenosis. Further cutting (repeat sphincterotomy) is sometimes

possible, but there are limits; surgical bypass may be necessary.

Attempts have been made to treat sphincter of Oddi dysfunction without

sphincterotomy-- by placing a temporary plastic splinting tube (stent), or

by an injection of botulinum toxin, which paralyzes the sphincter. These

treatments are experimental.

I am not having a problem with the blood pressure as I already had

hypertension. I just stopped my regular blood pressure med when I

started the Procardia. Procardia is hard to tolerate....I have had

many headaches but the longer I take it the better it gets as far as

tolerating it.

Dear Mark!

Thank you very much for your input. I have now quite

clear picture about S.O.D.

I suppose that basically it is worth to start with

antispasmodics. I am namely terrible afraid of all

what is going on in an acute pancreatitis. I've had

the first (hope the last)one in August this year.

Because I've read here that various abdominal

surgeries, esp. biliar, may provoke an AP I've refused

the ERCP/surgery thus far.

Zoie - it's luck in unluck regarding your hypertension

and therapy.

please allow me another question: besides headache

what are the other side effects of procardia which you

have noticed. flush maybe? ankle edema?

now on procardia - can you better tolerate the solid

food?

I could not tolerate the Procardia as I had the bad headaches as well. I had to

take Vicoden to get rid of the headaches !! Tried it for two weeks then I

stopped. My head hurt so bad that I had to hold it just to walk. Let alone turn

a TV on. It was awful. I don't know how people with Migraine headaches do it.

Color me a whimpy girl... I sure admire that you are toughing it out!
