Hi, Hollie:

I am the Queen of the Patient Information form, because mine didn't go

through after 8 tries. I only learned this by contacting Dr. R. and his

office. Check with them first.

If it didn't go through, print the entire form out before filling it

out...fill it out by hand. Then relog on to the Internet, and as soon as you

have, go to the PI form FIRST THING and begin filling it out. Try not to put

too much information into the description lines. That seems to affect its

going through.

One preop had to switch to another server, but I doubt you'll have to. The

only ones who seem to have real problems are those on AOL.
