Hi,

I also had a problem getting letter back after I submitted patient

information. After 1 week of waiting I emailed Dr. R and he checked

and said to resubmit. I resubmitted and did not get anything back

again. Emailed Dr. R. again and he wrote back identifying the problem

and what to do to fix it. So you can try emailing him. He was very

prompt in responding to my problem and once I made necessary changes

to my form and submitted again, I got my letter very soon.
