: I have Aetna HMO, and they approved within 48 hours, and paid

100%. The hospital is out of network, but they approved it as an in-network

hospital and paid the entire bill. Dr. R is in network, so that was no

problem. All in all, you end up paying a co-pay (mine was $20) for the clinic

visit, and that's all!
