Regarding the patient form, best advice is to first print it out; then answer every question as you want it to be . Go ahead, write it all in. Then, sit down and type what you've got written there. It goes a lot faster. Then you may actually succeed. (Some people have had to change servers, though.) Once you send that info, there is an automatic interface on the other end and your specific inofrmation is put into an already large, format letter. Then it is returned to you for you to print out and send off the insurance company. Trust me, you will get approval cause the medical necessity is spelled out. Proof the letter, be sure that it is correct, then send it. I needed to make a change, and I just simply wrote it in. I had no problems. Hope this helps.
