The ERCP is the only test that can definitely tell if you have sphincter dysfunction or not. Be aware though, the manometry (measuring of pressure in the ducts) can set off an acute attack of pancreatitis. It always has for me anyway. Also if you decide to have an ERCP, make sure that they test both the bile duct pressure and the pancreatic duct pressure. My first sphincterotomy (cut) they only checked the bile duct. 5 months later the SOD symptoms returned and I then found out that the pressure in my panc duct is very high too. You might also want to ask about testing for Celiac Disease. Since you are concerned about auto-immune diseases. Celiac can mimic chronic pancreatitis. Unfortunately I also have Celiac too.
