Guest guest
Hi, Sara; this is my attorney's take on the situation: give a letter of medical treatment management consent to them, and, if possible, have his doctor sign it as well. Limit the time period strictly; ordinarily, with close relatives, there is no problem under the new HIPPA regulations, but you want especially to make sure that he is cared for in case of need to go for emergency care. In the casual and wonderful world pre-Hippa, one just transferred TEMPORARY guardianship and medical decision- making to a friend or relative--it has all changed. If in doubt, consult YOUR attorney--might be worth the peace of mind--Love to you all, n Rojas, wcf, mom of 3 adults, one wcf Medical Consent forms Hi Everyone, My husband and I will be attending a wedding in April in Las Vegas. We have agreed that it will be our last solo vacation for a while, so we are are really looking forward to it. Zach will stay with 's parents for a few days while we are gone. They know how to do cpt, enzymes, ADEKS, and Prevacid, so I am not worried about his medications. I am concerned that if there were an accident and Zach needed to go to the ER, would his grandparents need a signed letter from us to get medical care for Zach or would it be ok as long as they had his insurance card? I am giving them a typed list of all of Zach's doctors and phone numbers just in case anyone would need to be called. Can anyone think of other things I should do before we travel without Zach? Guest guest
I would write a consent form that included the numbers of where you would be staying so that they could call to verify. Guest guest
Good idea! Come to think of it, we did always do that! Guest guest
Sara, I was a Paralegal for an Attorney, and it was advised to me to have Medical Consent forms for my kids " just in case " I have the form that I could send to you if you want it. I would have to be notarized, but 's parents could keep it with them at all times and it would be a legal document. If anyone else is interested in the forms let me know, and I can send them individually, or I can post it to the CF Parents website. Guest guest
That is an excellent idea. I have them for my grandchildren,- as I have stayed with them or they with me for years,-- for one reason or another. It is just a safety precaution. Thanks you so very much.. Guest guest
Katy, I would love for you to send me a consent form. Zach's godmother is a notary so that would not be a problem. I do not know how to post documents on the cf parents site, but if there is a way to do it, I think others would find this useful. Thanks!
