Guest guest Posted August 18, 2005 Report Share Posted August 18, 2005 Ironically, I have the opposite problem. I was able to compensate pretty well with my misaligned jaw. Now that I have a normal bite and can finally open my mouth more, I have trouble speaking clearly. My therapist pointed out that I still speak with my teeth clenched together but I don't know what sounds to open my mouth for. I have an underbite and just got my braces on last Tuesday, so my speech won't be getting better any time soon. My Surgeon told me that if I only have the BSSO for mandibular set-back, that my tongue will be too long for my mouth after the surgery because he will be making my mouth shorter by 7mm. This, he says, will create speech problems after I have been desplinted. I think that's enough reason to let him do the LeFort procedure for maxillary advancement, but he won't know until six months from the surgery what he will need to do. So, hopefully, I will need the LeFort; the whole tongue issue has me rather concerned. I know my speech has gotten worse since these braces were put on last week, especially in light of the fact that my teeth have already started to move. Both my OMS and my OD have told me that both my speech and my mastication will become worse as I get closer to my surgery - what a cheery thought! Also, if I have only the BSSO my tongue will be longer because my OMS will be shortening my mandible, and consequently, making my mouth smaller, by 7mm. But, if he does both the BSSO and the LeFort, my tongue will only be longer by a minuscule amount and won't be noticeable. Just what I need: to go through all this just to have a new problem develop! My OMS won't know until Dec. or Jan. if he will need to do the LeFort, although I've told him that I'm okay with that procedure and to do whatever he needs to do. It's sort of ironic: NOW I want him to do the LeFort; last month I was too terrified of it to let him do it! Hey, I have thirteen weeks of short-term disability time racked up and I want to use as much as I can! (Actually, my OMS says I will need a minimum of four weeks off if having both done.) Guest guest Posted August 20, 2005 Report Share Posted August 20, 2005 Hi Diane, I don't have trouble with specific sounds, but I tend to trip over my words. my tongue is too big for my mouth space, and I don't have good tongue mobility as a result. I also have a very deep bite, so that and the overbite and insufficient lower jaw, my tongue has no room to work to make the sounds if I need them to be rapidfire. I also have no room for food in my mouth when I eat. I have to pocket the food in the cheeks and feed very small amounts into my back molars to chew. The reduced space doesn't allow my tongue to mobilize the food well either, so the result is that I choke really often. Not so much on food, as I have always eaten this way, and am always extremely careful. It is just a way of life for me and I have never known differently (it is one of the things I am looking forward to post op). But if I get engrossed in a book, am studying, or doing anything that requires me to concentrate (and become absorbed in the subject), my jaw/tongue/cramped space causes me to choke frequently. I just can't handle fluid in that position. Sam - I support you in checking with your OS again about this. If you don't have a deep bite, perhaps it won't be a problem for you, but as a person who has a tongue that definately is too big for my mouth, I hope your OS can give you more information about what will be an acceptable tongue mobility spacing. If you have good space and volume in your mouth up vertically, perhaps it won't be as much of an issue for you, but I would be concerned.
