Help appealing United Healthcare

I am seeking anyone who has any advice on successfully appealing United

Healthcare for coverage of the DOC band. Like many others, it was denied

immediately and they requested more information. After I sent them a 13 page

letter that I wrote citing various medical studies and court cases, they

responded saying that it was not covered. It seemed like they hadn't even read

my letter at all, judging by their response. Our clinician agreed. I don't

know where to go from here. I know for a fact that UHC has covered this

treatment before.

Any advice would be greatly appreciated!
