I live in CA and also have Health Net. I have completed the form but have not

submitted it.

Health Net has much higher requirements to get the band covered than other

insurances. They want to see at least 12mm before they will consider covering

it. You can contact HN and get the specifics on what they require.

This isn't much help but we can work on it together.
