The first step is to read the insurance help files on this site. They tell you step-by-step what to do. Also, you can use the search function to look for old posts about Cigna and see if others had luck. I also always recommend looking up the laws that cover your insurance plan. Find out which laws cover your insurance plan. Email your insurer and ask if the plan is covered by state law, or what other law it's covered by. (Do all communication with the insurance company in writing, and save everything.) Often, there is a law requiring insurers to cover congenital/birth defects, craniofacial deformities, etc. This can help in your appeal. Also, read other people's appeal letters to get ideas. Make sure you know 1. the reason for the denial of coverage, and 2. the exact language of the part of the plan that they say allows them to deny coverage. Cigna didn't deny the claim immediately. They sent a letter saying they needed more information and then once we sent it they said that our plan doesn't cover it. Which makes us wonder why they didn't deny the claim the first time. If anybody has had luck with this please let me know.
