My daughter wore a starband starting at age 5 mo, and it was definitely a good choice. I would suggest you look at your child's head from the top and try to see if is just a flat spot on the right. Sometimes a different view helps you see more asymmetry than when you look at your child daily. Take pictures and/or look at him in the mirror to see if it is mild. At 3.5 mo you have time to try to reposition him. Keep him from resting on the flat spot as much as possible. Have him rest on the prominent side (should be opposite the flat part). Use a baby carrier or bumbo chair and keep him out of carseat (except while in the car), bouncer, swing etc. This is called " repositioning " .
