I would modify it so under 'notes from teacher' it breaks down parts of the day: in circle we read..., I played with.....(toys or friends), my target words are ....., Etc. as needed for your child's school. Also, if u ask the school to laminate it, they can write w/a crayon & u can wipe off. Only do this if they won't complete it each day.
