To Whom It May Concern:

I am a student at the College of Saint Rose studying Communication Sciences

and Disorders. I am currently working with Overby, Ph.D., a professor

at the College, to develop a study investigating the common needs of parents

of children with Childhood Apraxia of Speech. Counseling and supporting

families of children with communication disorders is a recognized need.

However, little information is known about the social and emotional needs of

parents of children with CAS. We have developed a parent survey that we hope

to use to identify those needs.

I am writing to request your permission to invite members of your parent

support group to participate in the survey. The survey will be available

between December 8, 2009 and January 5, 2010. It will be available

electronically through Survey Monkey online survey tool

(www.surveymonkey.com) and will take approximately 10-15 minutes to

complete. Participants will send responses directly to the researchers. You

will have no responsibility related to the survey other than providing the

information and survey link via forwarded e-mail or by posting it on your

website. No identifying information such as name, address, or geographic

location is requested and all survey responses will remain confidential. All

survey data will be encrypted. Participation in the survey is voluntary and

all participants must be 18 years or older to complete the survey.

Participants are not required to respond to any questions that they do not

want to answer. Participants are free to withdraw from the study at any

time, even after beginning the survey.

The research project has been approved by the Institutional Review Board at

the College of Saint Rose. If you would like to extend this invitation to

the members of your parent support group, would you please notify the

researchers by responding to this e-mail? You may post the link starting

December 14, 2009 for up to five weeks until January 11, 2010. Posting or

e-mailing the survey invitation and link will qualify as implied consent to

participate by the support group administrator.

You are free to examine the survey before deciding whether to e-mail it to

support group members and/or post it to your website. If you are a parent of

a child with CAS, you are also invited to respond to the survey.

An invitation to participate is attached to this e-mail. The invitation may

be e-mailed to members of your parent support group or posted on the support

group website.

Childhood Apraxia of Speech Parent Survey

You are invited to participate in a study examining the common needs of

parents of children with Childhood Apraxia of Speech (CAS). The study

consists of a survey that will take approximately 10-15 minutes to complete.

The survey is available electronically through Survey Monkey survey tool.

Counseling and supporting families of children with communication disorders

is a recognized need. However, little information is known about the social

to identify some of those needs. No financial compensation will be provided

for completing the survey, but we hope the results will help researchers

understand how to support families of children with CAS.

Information collected in the survey is confidential. No identifying

information (i.e., age, address, geographic location) will be requested. All

survey data will be encrypted so the researchers can not identify you.

Participation in the survey is voluntary. Participants are not required to

respond to any questions that they do not want to answer and participants

are free to withdraw from the study at any time, even after beginning the

survey. Participants must be at least 18 years old to complete the survey.

There are no known risks associated with participating in the study.
