Forward - Sharing: McGill Early Intervention Survey

~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~
Dear Parents,
We would like to request your participation in a National study of Early
Intervention for children with developmental delays. The purpose of this
questionnaire is to survey parents of children with developmental delays from
across Canada to determine what they believe to be the critical success factors
and current gaps in service delivery. Responses will be used to make cross
province/territory comparisons in an effort to facilitate a national
paradigm shift toward effective Early Intervention (EI) across Canada.
Parents are being asked complete a brief questionnaire used to assess their
experiences with EI, coping strategies, and perceptions of family-centered care
in their respective province. Participation shouldn't take longer than 15-25
minutes.The survey can be completed online at
http://www.zapsurvey.com/Survey.aspx?id=e09fe4d1-7ff6-4926-b55a-1c88deb7a4f0
If you have any questions concerning this survey, please contact
Saracino, Primary Research Associate, at jennifer.saracino@... She
can email you a copy of the questionnaire, set up a time for a telephone
interview, or fax
the questionnaire to you.
Sincerely,
Saracino, PhD Candidate
