Are there any data collection sheets food/behavior out there?

Before I try and reinvent the wheel, has anyone come across any worksheets that

are set up to help you collect behavioral/sleep data on your child as well as

what food they are eating. I know they've got some for doing a functional

behavior analysis (is that the name?), but I'd like one that goes a little

further.
