Ask for verification! Mistakes do happen and you have the right to an accurate report! You also don't want false info going out to other schools. Happens all the time. Have emailed the guidance officer asking where she got that from as we weren't told any such thing by psychiatrist. Then you can piece together what is going on. I have had things added to reports and evaluations before and had to make them fix the mistakes. Sometimes they have even put the wrong name in, age, sex, etc. Always get the report and always check it over so that it really reflects your child.
