The first one I don't remember ever signing for her but maybe I have but I know I have with my sons ifsp, it is a release of medocal records and medacaid release. However the 2nd is a physical form. Stating the child must have a complete physical from a licensed physician but doesn't state a time frame. She had her checkup in march and her physical with the dev ped in sept so I assumed one of those would work. But it asks for dx and dx date, if they agree that sped is appropriate placement and any accomodations they feel is needed so I thought the dev ped would be most appropriate to sign this. Has anyone seen a form such as this. Will a faxed copy be ok, the dev ped is not nearby. And what do they mean by is sped an appropriate placement? Is that in case she goes to resouce? Or is that what any iep means?
