I was told by a PS in Salt Lake City that doughnut (concentric) mastopexies don't work. I did not believe him though (he was not exactly a classy PS). I don't know why more PS don't use the concentric technique--it would seem that this is better but don't you need the vertical cut to achieve projection?

Hi Ladies,

Based on my many hours of conversations with a few of the 'top' BI / Explant PSs in the DFW area ..... it takes an extremely experienced PS to perform 'the perfect' donut concentric technique explant/uplift procedure. Dr. Melmed, in Dallas, is one of those PSs, plus he has developed yet another 'criss-cross' version of this technique for certain problems of possible extreme 'sagging'!

They are not ideal for breast ptosis without implants but the particular defect that occurs with most implant patients is one of excess skin in a particular pattern that is often best treated with a donut mastopexy. The vertical component for projection does not always work with explants. .
