I am curious about this too, would this be enough to really lift a saggy breast and if so why are vertical mastopexy's so popular?

Anyhow I cannot hardly find my vertical scars, the ones around the nipple are the easiest to see of the scars I have, but those are smooth, fine, and white.

Hi Dr. Kolb, I saw your recent post about the doughnut lift and vertical lift with explant. Would you kindly tell me if just a dougnut lift is performed does the breast still get a "lift" (lifted) or is just the excess skin tightened up? Thank you. J- "Dr. Kolb" wrote:

I had five bouts of telogen effluvium following my implant/explant surgery, crash diet, and going hyperthyroid on too much T3. I lost about 50% of my density--it totally sux. It's slowly coming back somewhat but I feel I need more medical attention to get full regrowth underway. It's important to have the body cleansed out as well as having adequate protein and iron stores in the body. Perhaps Dr. Kolb can tell us more about this as this is a devastating thing to deal with. Why do you think that the vertical mastopexy is so popular? Most surgeons that I know use the donut mastopexy as patients do not want the vertical scar which is usually much more visable. .

Thank you Dr. Kolb. Would you kindly tell me if explant is ever performed on an emergency basis? I am in a terrible situation and in need of advise. My surgery was cancelled last Monday due to a secondary respitory infection. The surgeon (original implanting dr) is angry and does not want to reschedule me. I waited months for the apt. He was doing explant only and general surgeon was removing lymphs. The only other surgeon in my area, I trust says that my case has progressed too rapidly (I have old meme under muscle with extracapsular rupture and migration and lymph node involvement) and I am spilling at unusually rapid rate and that my surgery is "urgent" and "severe" and must be handled "immediately" and can not wait until she has availability - that it would be a "severe health risk". I am wondering if explant is ever so urgent that it needs to be done on an emergency basis? I am so sick and have no surgeon at this point and I do not know what to do. Can you please advise me? Thank you very much. J- "Dr. Kolb" wrote:

From my understanding the vertical lift technique that Dr Feng uses is better than a donut lift. I know it worked for me and if you have seen my photos then you can tell that I was very saggy. The PS's that I have talked to about the donut lift all say that it is limited in how much lift you can get when dealing with extreme sagginess and loose skin. I guess it depends on the patient and the breast skin etc. I am happy with my vertical lift and like I said I have to really search to even see the scars at all.

I am not sure why the vertical lift is so popular, other than it would give the best outcome?

I know of only one other woman who got a donut mastopexy, and she was not too happy with it--she mentioned the puckers around the nipple area, like when you sew fabric and there's too much of it...I never saw her....can only go by what she said.

Puckers in the nipple areolar area can occur early but usually fade with time. If the puckering is too much then a vertical lift can be done. Each patient has a unique defect. Some need a donut, some a vertical, and some an anchor pattern. See any plastic surgery textbook. .

: Each patient has an individual defect. One lift is not always better than another for everyone. Most plastic surgeons will taylor the skin enevlope for the best result not only use one technique. I recently had to redo one of another plastic surgeons patients with a significant deformity who should have had a donut mastopexy but didn't. The correction was a donut mastopexy. One size does not fit all. .

I agree with this statement it would be obvious to me that one needs to be treated individually. In my case I know I needed the vertical lift to get good results, because I needed it before implants, and certainly the problem got worse with the 390cc implants I had in me over a year.

Now I am pretty happy with my breasts, and even get dressed in the locker room at the spa. I feel like I have the youngest looking boobs around on a 38 year old that has had a kid and allot of weight gain and loss.
