It is good info and very helpful. I have textured saline, and I was wondering about a part you wrote in there about capsuels and what it meant...... The most difficult explants are around textured and polyurethane implants as these are often vascular capsules and also in patients with virtually no capsule and extensive migration of silicone in all directions. Those patients who have capsule failure are also more likely to have systemic illness. What is capsule failure? Is this a problem with the capsule(like a tear), or failure to form one completly? If there is very little capsuel, and it is vascular, is it better to just leave some behind? Is this a problem with textured saline(shell breaking down), or is this only important with silicone gel? Guest guest Posted May 15, 2002 Report Share Posted May 15, 2002 Darlene, Though I am not dr Kolb and not clear about some of your questions either, I do know that if you have textured implants and esp if you are having health problems as you are, there is no way that you would want to leave any of that capsule behind, I believe that due to the fact it becomes vacularized this can cause the chemicals to leach out into the bloodstream which would account for our toxic state and our symptoms, of course it is not the only explanation for our illness but it is probably one of them. I hope that you will make sure when you get your explant that you go to a PS who is willing to remove all of your capsule no matter how tedious and difficult it is this is so important if you want to be healthy again. Remember that the capsule can harbor particles that may have flaked or sloughed off and that it can be just as bad and ilicit an immune response like the implant itself can. I think that capsule failure is probably referring to silicone gel patients who had gel leakage that got outside the capsule, so that would not really apply to those of us who only had saline. I worked at Mills Hospital in SM and then at Stanford in Palo Alto! Did you grow up there ? How long have you lived there?
