And you have confirmed this explant will be covered? I completely understand your dilemna about "good" and "bad" insr. No one wants to take medicare/medicaid because the reimbursement is so poor, but it doesn't give doctors an excuse to deny or "turn away" pts from getting care they need. I ask the same question as you--why does it seem that people with "poor" insr are worse off than those w/o insr? I don't know that we will ever have those answers. All I can say is that our health care system is one of the worst in the world as far as the insr issue goes, and it's only getting worse. And what pisses me off even more, is that I was just on a website this past wknd where it reviewed average salaries of MD's, nurses, and pharmacists. The salaries would astound you--even me, who is a RN, and has some knowledge of what docs are making, well, it blew me away. After seeing these salaries, you'd think that a few charitable cases a yr wouldn't be a big deal. But it comes down to greed, and the almighty dollar! Anyway, I have some ideas about your situation. First of all, I would like to try to clarify what would be classified as an "emergency" explant. Now, Dr. Kolb or anyone else can correct me, because I could very well be wrong, but based on my own personal situation and what Dr. Huang told me a yr ago, is that an emergency explant would be any case in which your health is endangered, or you are experiencing a severe reaction, in which waiting for any period of time could be detrimental to you. I can't remember the exact circumstances of your situation, but if I recall correctly, I remember you writing that something major had happened (a rupture?). And do you have silicone or saline? How long have you had them? What health problems do you have? The fact that your doc is willing to support you with a letter is a plus, now it is just finding a doc who accepts your insr---correct? And what state are you in? I can see what I can do when I have the answers to the above questions. I will honestly do what I can, to see what info I can obtain for you. Medicare primary and Medicaid secondary (which would pick up the co.-pay). That is important because I will require a hospital stay (and a bill for that too). But ps is a whole other story............a general surgeon could do the lymph's simultaneously. But the silicone spill has to be addressed by an experienced ps. I have severely ruptured old MEME (Polyurethane) SILICONE under the muscle with extracapsular rupture, migration outside of the capsule, and silicone seen in the axilla lymph nodes and most importantly I have calcification in the lymph node in my groin. plastic surgery indicated it was silicone but My rheumy thought surgery ALSO explained to me that if the " calcification " in the lymph in the groin area is NOT silicone then it could be a lymphoma (which is cancer). There is no way to know until its removed and checked. I did not know this until a week ago. Because of my cardiac and pulmonary status undergoing 2 surgeries is NOT recommended. NOW also a minor problem on my upper arm is a BIG problem that they are evaluating. He is looking for " silicone debris " I have a lump and pain, now numbness in my fingers. Just great huh? They think it is part of the " shell or something " I can see it on the MRI films myself. I could see the progression of the extracapsular migration myself and I have major increase in localized symptoms. It is just going to get messier and more complex the longer the delays. I have severe systemic medical problems (that is why I am on disability) as well as localized problems (SEVERE burning / pain). Most likely from the raw silicone in my tissue and the inflammatory response!!! My primary dx is lupus with all the complications - secondary dx are too long to list. I have pulmonary, cardiac, vascular, skin, muscle, bone, ect..... involvement I have known about the rupture since FEB and STILL can't get this out!!! I keep trying and trying......Everything is getting worse!!! Obviously, no one is suppose to have silicone " spilt " in their body. It is a " medically necessary and urgent procedure and not elective surgery. I need a very good dr. I guess I could have gone to any " butcher " at some clinic somewhere, but my case definitely warrants a SKILLED and EXPERIENCED surgeon. I can not undergo several surgeries!!! Calling the insurance co. for a referral is joke, as they would probably recommend a clinic (i.e. the cheapest way to go)......Yah right like their going to know how to handle a silicone spill outside the capsule and lymph involvement and potentially lymphoma on a Lupus patient!! Calling the American Board of Aesthetics and American Plastic surgeons is also a joke, as they just have a list of board certified surgeons. Spilt silicone is not exactly listed as a specialty or on their CV. Who would pick a dr. out of a hat like that for this type of procedure? You would have to be totally ignorant. I suppose, ignorance is bliss. And then there is the insurance and financial issues...... As you say and as I know all to well from this experience.......... try to find a dr. willing to accept my insurance. It is really a catch isn't it???!!!!???? I hope I have answered your questions. I have contacted her to see if she does reduced fee surgeries or could help out in any way. I'd like for her to be able to read what your health problems are and what implants you have. I know that if Dr. Huang pulls through on this, and you can get out to Denver (I actually have a companion ticket I might be able to give you on American Airlines), I can help you out even more. The salaries would astound you--even me, who is a RN, and has some knowledge of what docs are making, well, it blew me away. After seeing these salaries, you'd think that a few charitable cases a yr wouldn't be a big deal. But it comes down to greed, and the almighty dollar! Anyway, I have some ideas about your situation. First of all, I would like to try to clarify what would be classified as an "emergency" explant. Now, Dr. Kolb or anyone else can correct me, because I could very well be wrong, but based on my own personal situation and what Dr. Huang told me a yr ago, is that an emergency explant would be any case in which your health is endangered, or you are experiencing a severe reaction, in which waiting for any period of time could be detrimental to you. I can't remember the exact circumstances of your situation, but if I recall correctly, I remember you writing that something major had happened (a rupture?). And do you have silicone or saline? How long have you had them? What health problems do you have? The fact that your doc is willing to support you with a letter is a plus, now it is just finding a doc who accepts your insr---correct? And what state are you in? I can see what I can do when I have the answers to the above questions. I will honestly do what I can, to see what info I can obtain for you.
