Sometimes it is not possible, then they must do it the way you are describing, however, they should first at least attempt to do it en bloc. The reason is really obvious why, esp when you have silicone gel, you want the implants to come out whole for two simple reasons really, first to protect your body from having the implant contents leak or drain into your body, and second to get as much out the first time, so they don't have allot of digging around to do. I would tell this Dr Nice that he needs to at least promise in writting that he will attempt to get them out enbloc. If he cannot then at the very least he needs to remove the entire capsule. SInce you don't have saline it is not possible to drain the contents first and then remove them, I have heard of this with saline but it is not possible to drain gel.
