St. 's Wort, Life Everlasting (Cudeweed), Gotu Kola, Rice Brain, Lady's Slipper, and Kava Kava. Supposedly these herbs in combination have done some good work on ADD among other things. The licorice and ginseng will help with his adrenals and balancing out his hormones and their precursors. You might also consider using the teas and making a smoothie out of them. However, for a 5 year old don't use any Kava. Ginseng and licorice with a little ginger can be very useful. This can be made with ACV but as licorice, ginger, and ginseng are all roots you would be better off using alcohol to tincture. You can then add the tincture to some water and heat it to drive off the alcohol then add in some honey to sweeten and a little glycerine to preserve it. If you have a juicer, lots of fresh organic fruit or veggie juice will at least give him the nutrition he needs while there. It is too bad his parents won't do the same for him at home.
