OT: question about DDI UTM tests and insurance

Has anyone had their insurance pay for UTM tests from DDI? Our old

insurance paid nothing out of network and nothing from their specific lab. We

now

have GHI and out of network benefits. I'm wondering if its worth putting their

info on the back of the form when I send in the test? Or is it better to pay

the $99 in advance and submit later?
