The flesh penetration of narrow bands of light is very shallow so it will rarely be a first-line therapy for advanced cancer. I do think that PDT is underutilized. I spent many hours discussing PDT with the CEO of a company in China that makes the equipment. I think that we would see better results if a number of targeting porphyrin/chlorophyll/heme analogs were used along with a variety of lasers. I may make some of these compounds but right now there are not enough hours in the day.
