Guest guest
Hi, I have been on HCG monotherapy for secondary hypogonadism since April 1, 2009. The symptoms that led me to investigate and diagnose my condition include zero libido, always low energy, inability to lose weight inspite of strenuous exercises, inability to gain muscle, slight gynecomastia and depression-like mood. These symptoms have been there for a very long time now, close to 10 years. I am 29 years old. I wouldnt rate myself as obese - I weigh ~190lb and 6'1 " tall. My body composition seems to be more fat than muscle. Body fat is about 24%. My problem is libido. Since the hcg therapy started in April, I have felt some positive changes - a slightly better sense of well being and better mood and energy. I definitely see an improvement in my weight lifting capacity. My ED issues have also improved significantly. However, the one problem that is still has not shown any improvement is my libido. I have absolutely no libido - no change from how I felt before the therapy started. I have pasted below some of my blood test numbers. If you have any idea what might help my condition, please let me know. I have tried Cialis too and it did not help. My doctor likes me to stay in the 500-700 range for testosterone and is satisfied with my latest results. He thinks the libido should slowly improve. But I see absolutely no signs of improvement since the therapy started 3 months ago. My current therapy details - HCG monotherapy start date - 4/1/2009 HCG current dosage - Pregnyl 1500units twice a week IM I am also taking Altraz (equivalent to Arimidex) - 0.5mg twice a week, on day of HCG injection since 6/1/2009. My gynecomastia has improved - about 50% reduction in ~3weeks time. I have not been taking Altraz since June 30 - I dont have a current blood test to note my E2 and afraid it would plunge too low. Will restart it next week at 0.25mg twice a week. Reason for staying on HCG and not doing Testosterone injections - I am 29 and afraid it may lead to fertility issues (my sperm count was already on the lower side before therapy started). I know that a mix of T and HCG is possible, but havent considered trying that yet. Date Total Test. Ref: 300-1200ng/dl Free Test. Guest guest
The test results got a little out of order at least on my screen. I just have two observations: I would figure you to be pretty skinny at 190lbs. and 6'1 " . Your TSH looks very high and should trigger further Thyroid testing. ________________________________ From: Ashok k Narasimhan <ashoknarasimhan@...> Sent: Saturday, July 11, 2009 1:32:35 PM Subject: hcg monotherapy, no libido Hi, I have been on HCG monotherapy for secondary hypogonadism since April 1, 2009. Guest guest
The first thing I need to tell you is doing 1500 IU's 2x's a week is way to much at one time. Dr. Johh says at www.allthingsmale.com in his HCG Update not to do more then 500 IU's in a day. If you do more for a long time it will deseatize your testis to the HCG that acts like LH and you will become Primary meaning your testis will stop working. Best to not do them high dose's the are for women trying to make a baby. Cut back to 100 IU's a day to 200 IU's per day for starters. You just might find your levels will stay the same. You see when you do more then your testis can handle it converts into estradiol and what your taking looks to me like it to much if you don't get night time or morning wood your Estradiol is to low and taking a pill will not help at all. Stop the Arimidex like drug until your wood starts up then do .25 mgs every 3 days. If your levels start to go up then do it every 2 days if you get sore and hard nipples best to keep Estradiol at about 20 pg/ml. When I started on HCG when after being told I am Primary for 23 yrs I add HCG to my T shots 500 IU's 3x's a week. My Total T levels doubled and my testis at the time were the size of small grapes. I cut back on the dose and went to doing my T shots every 3 days doing the HCG 400 IU's the 2 days in each in between my 70 mg T shots. Cutting back on the HCG my levels stayed the same and my Estradiol levels came down. I am not sure what to tell you to do about your estradiol at the time I was doing a big T shot once a week and doing the 500 IU's of HCG every 3 days it took me taking one mg of Arimidex to keep my Estradiol down. I am thinking your levels must be to High but then again they could be to low. I would do this get labs down talk to your Dr. about doing your HCG less everyday and after you do your labs cut back on the HCG to everyday. See if your ED stops and you get night time and morning wood. I would not stop what your taking for your Estradiol unless your test comes back to low. Do the Quest #4021 test it has a range of 13 to 54 pg/ml you need to be under 30 or at 20 this is the best levels for most men. Guest guest
Thank you Phil and for your replies. I will talk to my doctor about the desensitization concern. I'll find out about the Quest E2 test. About the AI I take - even when my E2 was at 70, I did not feel significant high E2 effects. For a very short while my orgasm timing was disturbed, but it returned to normal automatically with out the AI. I had weak night time erections. Since I started the AI, the erections have become stronger definitely. Right now I thought they were a little bit weaker. Hence stopped the AI. I was hoping that high E2 was the reason my libido was down and taking AI would fix it. After 4 weeks of the AI at 0.5mg twice a week, I am sure my E2 would have dropped a little bit atleast, enough to make an impact on the libido. But it hasnt yet. I am due for a blood test in a couple of weeks. Apart from the usual ones shown here, is there any other tests that you suggest I should do, which might help get a better understanding of the situation? Also, I am re-posting my lab numbers so you can see them better - Date : 1-30-09 Total T: 293 ng/dl Ref: 1.8-3.9ng/dl Free T : 0.76 ng/dl Ref: 1.8-3.9ng/dl E2 : NOT TAKEN Ref: <20-55pg/ml SHBG : NOT TAKEN Ref: 13-71nmol/L Albumin: 4.3 g/dl Ref: 3.5-5g/dl Date : 2-25-09 Total T: 247 ng/dl Ref: 1.8-3.9ng/dl Free T : 0.73 ng/dl Ref: 1.8-3.9ng/dl E2 : 48 Ref: <20-55pg/ml SHBG : 23.4 Ref: 13-71nmol/L Albumin: NOT TAKEN Ref: 3.5-5g/dl Date : 4-14-09 Total T: 466 ng/dl Ref: 1.8-3.9ng/dl Free T : 1.50 ng/dl Ref: 1.8-3.9ng/dl E2 : 70 Ref: <20-55pg/ml SHBG : NOT TAKEN Ref: 13-71nmol/L Albumin: 3.5 g/dl Ref: 3.5-5g/dl Date : 5-15-09 Total T: 633 ng/dl Ref: 1.8-3.9ng/dl Free T : 1.57 ng/dl Ref: 1.8-3.9ng/dl E2 : 71 Ref: <20-55pg/ml SHBG : 23.2 Ref: 13-71nmol/L Albumin: 3.5 g/dl Ref: 3.5-5g/dl Thanks, Ashok > > > From: Ashok k Narasimhan <ashoknarasimhan@...> > > Subject: hcg monotherapy, no libido > > > > Date: Saturday, July 11, 2009, 2:32 PM > > Hi, > > I have been on HCG monotherapy for secondary hypogonadism > > since April 1, > > 2009. Guest guest
Actually I forgot to include one other test - the high TSH was in Jan 2009. Subsequently I had another TSH done in 2 months time and that showed only 2.7 mcunit/ml. The Free T4 remained about the same at 1.7 ng/dl. I had Thyroid antibodies also done, which showed everything to be normal. So that line of investigating the thyroid function was dropped. Thanks, Ashok > > The test results got a little out of order at least on my screen. > > I just have two observations: > > I would figure you to be pretty skinny at 190lbs. and 6'1 " . > > Your TSH looks very high and should trigger further Thyroid testing. > > > > > ________________________________ > From: Ashok k Narasimhan <ashoknarasimhan@...> > > Sent: Saturday, July 11, 2009 1:32:35 PM > Subject: hcg monotherapy, no libido > > > > > > Hi, > I have been on HCG monotherapy for secondary hypogonadism since April 1, > 2009. Guest guest
The thing about TRT is one needs to get leveled first for most this means getting Total and Free T levels up into the upper 1/3 of your labs range and keeping Estradiol down to about 20 pg/ml. Then when your there and feeling better your body starts to undo the damage done by low T and you get better a little everyday. The libido is the last thing to come back and for some it does not get all the way back. So you need to make it a point to have sex once you start all is great.
