Guest guest Posted July 10, 2009 Report Share Posted July 10, 2009 You are lucky your Endo is good she knows what she is doing. Your MRI is to look for a tumour or damgage to your Pituitary Gland. I have this problem do to a head injury took me 23 yrs on TRT and not feeling 100% to figure this out. Don't worry if you have a tumor it's very very rare to be cancer and the treatment is the same. Your having ED problems because your Testosterone levels are low and your Estradiol is high you need to get your T levels up into the upper 1/3 of your labs range and to keep your Estradiol E2 down to about 20 pg/ml getting this down you will get your night time and morning wood back and your ED will stop. But once one has ED there mind can sabatosh there having sex you worry about it staying up and lose it anyway so to over come this take Cialis get the new 5 mgs pill you take everyday this way you don't need to wait for the pill to work in time you will not need it. At the bottom of this link you can read about Pituitary Problems I have what they call Hypopituitary Problems. http://forums.realthyroidhelp.com/viewtopic.php?f=12 & t=256 If you read up on this you will see that other hormones might need treating like Cortisol, Thyroid, Aldosterone and Renin levels. We can go into this deeper later just do the MRI and let us know how it comes out. I had mine and it showed nothing but doing an ACTH stim. test showed me to be Secondary meaning my pituitary is not working right. Once you treat this you will feel you old self again. Sorry this is long but I wanted to make it as detailed as possible, thanks for reading. It's standard practice to look at the pituitary, when LH and/or FSH are low - or if prolactin is high. There are principal ways people can have low T. One when the testicles do not work - is designated primary hypogonadism. The other the pituitary is not sending enough of the right hormones to make the testicles do their job. This is called secondary hypogonadism. They need to be sure there's no tumor on the pituitary. In most cases there is not. (If there was high prolactin it would be more likely - not so in your case. ) Relax, the odds are way way in your favor they will find nothing. But they need to check this before starting T therapy. It sounds like you have a good endo. SHe's testing for E2 and doing the MRI before prescribing. It's amazing how many fail to follow these best practices. Yours is doing what she should. Read this document to learn the basics of what you are looking at. It's a little short on information on E2. I have a bald spot near the back of my head and my hair on top has been thinning the past few months. I still have a decent head of hair but started becoming concerned the past few months. I noticed my hair was falling out months ago but only noticed about 2 months ago I had a bald spot. I assume a person with primary hypogonadism would probably have symptoms like undeveloped genitals, lack of body hair? In my case I think I'm secondary because the Endo told me my body hair pattern was normal and my testicles/genital size is normal. Is one particularly worse than the other? What kind of therapies are available to help combat this thing? My chief concerns are mainly getting my energy back and my mind straight and not muddy so I can move forward and do something with my life since I'm stuck at the moment and I would also like to someday meet a special person and hopefully have children of my own..I don't know how this problem will impact that aspect. If one is Primary it means his testis are not making enough Testosterone and his LH and FSH levels come back high near the top of the range or higher with low testosterone. Secondary the LH and FSH is the message from the pituitary telling the testis and Adrenals to make more Testosterone so most men with this problem can get and keep there testis working taking shots of HCG 100 IU's a day to start. But before you do any med like HCG or Testosterone have your sperm checked if it's good get some forzen just for a back up in case later you want a baby and can't. Sleep apnea is emerging as a common cause - evidently decreased oxygen flow - and the areas of the pituitary that produce each hormone are very small so even small impacts add up. Primary (testicle failure) can be from birth or develop later in life - eg. caused by the mumps, twisted testicle, etc. If the it is prepuberty you don't get full puberty effects. This is probably the harder condition as these folks sometimes do not get puberty penile growth, etc. Treatment is mostly the same - getting testosterone in the body. You will get your energy back. And strength and ambition and motivation. You're going to be blow away by how good it feels to feel good. It's crucial though that your doc monitor and manage E2 levels. As these rise - and they will with most T therapy, they rob you of the effect of the T. (T is converted by an enzyme into E2. ) Testosterone replacement therapy can cause temporary sterility. There is actually research to develop male birth control by using T. There is another hormone HCG. http://en.wikipedia.org/wiki/Human_chorionic_gonadotropin You can raise T levels with this and be fertile. Some docs supplement T with HCG when people are trying to get pregnant. Some supplement generally because it keeps the testicles working and helps them keep their size. (When you take T your balls stop working and for many folks shrink a bit. Unless you take HCG.) HCG is tricky though, if you take to much you can burn out the testicles. Read up. You need to get educated on this stuff and learn for yourself to protect your interests. Lots of the docs do not know this stuff very well. The reason I asked about balding is some guys take propecia to keep or grow hair and it fucks up their hormones and they end up on T for life. Get a doctor of naturopathy (not just a herbalist or naturopath. My advice, given your youth , is to go organic. It turns out that my Pituitary is normal, they found nothing wrong with it. The Dr's assistant told me over the telephone because my T levels low-normal the Endo wants to hold off on Testosterone Replacement and wants me to get another Blood Test in 3 months. I like this course of action because if I was offered it anyway I wanted to take time to see if Lifestyle changes could have some kind of impact because offer the past few months I have been very stressed and have barely gotten any sleep, went to sleep at different times because of this. I have been majorly depressed over it. So I don't know what I'm going to do now that I got my results; there is nothing wrong with my pituitary, yet my T Level's are still low. With high stress it climbs and can cause low T. But if you're cortisol levels are normal and T is not up, I'd get started on T sooner than later. I can't afford to lose any more time and nothing will be accomplished waiting 3 months to learn what I already know...that my testosterone is low. Do like Reto says get a morning fasting Cortisol test if this comes back below 15 then get a 4x's in a day Saliva test you can buy this your self from this link. Cortisol and E2 should be tested before starting TRT. They've already done prolactin, right? Look for Dr. Crisler's paper in the files section. Guest guest Posted July 18, 2009 Report Share Posted July 18, 2009 Hi, You've mentioned that, you were very much stressed and had very little sleep. This may be the main cause of your Low T. I was in the same situation and my T came as 260. Just like you, my MRI for the Pituatary Gland came clear. My Endo stupidly prescribed me Gel which messed up my T level to the extent that I lost my job. Anyway, to make it short, in your case since the MRI is clear, you can give yourself some time with proper sleep ( 10PM to 6 AM is the best time for sleep !) , Good Diet ( Net is good resource to find diet which would help to increase your T naturally! ) and Moderate , Regular Exercise . I opted for this alongwith Tribulus herb ( to stimulate the natural T Production ) for 4 months and my T is naturally back up to 352 from 260. I am still holding on to this regime and hopeful for T level of atleast 425. Each body responds differently to each approach, but you can sure try this as TRT is for Life and comes with lots hassle of meds and injections and ofcourse with side effects. Trying the above approach with a watchful eye about how you feel, whehther you have morning wood or not, your overall sense of well-being etc. is worth for 3-4 months of time. But here's the newest turn of events: I went to a Urologist today for this problem and some other things i've been experiencing and he gave me a prescription for a drug called Amiridex. Yes, Arimidex can help raise your Testosterone up a bit if you convert a lot to Estradiol. 1mg per day is way too much unless you are a women with breast cancer. 1/4 a tablet every 3 days to start is usually enough. Without TRT that dose may even be too much. Guest guest Posted July 22, 2009 Report Share Posted July 22, 2009 I forgot to mention, here are my new labs Total Test: 417 (250-1100 ng/dl) Free%: 3 (1.50-2.20%) Free Test: 125.1 (35.0-1.55 pg/ml) LH:3.8 (1.5-9.3 miu/ml) Prolactin:11.2 (2.0-18.0 ng/ml) The labs were the best one's I've done but I think it's because it was in the morning. I drew the blood around 9:30 or so. Still the Testosterone level is sort of borderline. The Uro gave me a prescription for the Amiridex. My estradiol level from a previous test 41. I understand Estradiol is estrogen. Guest guest Posted July 23, 2009 Report Share Posted July 23, 2009 We have a guy here that was on Androgel for some yrs. he seen a Uro because he did not feel all that great. The Uro took him off the Androgel and put him on Arimidex 1 mg. pill a day his body started to make it's own testosterone. As Estradiol levels go up your brain thinks it's Testosterone because Estradiol is made from Testosterone so your brain sees Estradiol high and slows down sending the LH and FSH messages to your testis and you can end up with low levels. Here is a link about this it's about older men but I see this happen to younger men. http://jcem.endojournals.org/cgi/content/full/89/3/1174 You need to be care full on this you can go down to low fast I did not see if your Estradiol was tested but if it's over 30 pg/ml this can happen I am on TRT and take .25 mgs every other day or 1/4 of a one mg. pill to keep my Estradiol down. And there are times I go to low I gage this by my night time and morning wood. When I went on Arimidex I had some bad ED problems and this got better fast the first thing to come back for me after not having them over 35 yrs was my night time and morning wood. So when it stops I know I am going down to low to low is just as bad as to high. Make sure you don't take this for months with out a lab test on it. But yes this works for some men. I have tried finding the posts but can't I think he posted to someones thread you can try and see if you can find it or someone might know the guys nick.
