Test Results and How to Pay ?

I asked my dr for a testosterone check since I had had a low normal

test a few years ago. It looks he ran a whole battery of tests and now

my insurance won't pay since he told them the tests were for ED.

I'm wondering how I can get them resubmitted in a way that they will

pay.

Anyways my results are as follows:

Free Testosterone .8 (L)

Prolactin 5.6

TSH 3rd Generation 1.61

LH 2.0

FSH 3.8

Anyways now I'm on 300 mg testosterone every 3 weeks. Does that course

seem appropriate?
