I was SUpposed to go for an MRI of my brain to check for the perceived presence

of a Tumor in my Pituitary Galnd..However, upon further evel by my regular(new)

endo and Primary Doc they both said the elevation of Prolactin in my blood test

is slight and a result of other meds I am taken

Hi Matt

Sorry to hear of your medical difficulties and blood test results.

I was first taken ill at ca 17 but did not have (late 1960s) full blood tests

and so early

symptoms were missed. However, the symptoms you describe and blood tests need

far

more investigation and unfortunately this can mean still waiting for treatment

so as not to

mask or interfere with blood tests and investigations. Your Endocrinologist

and other

members here will report that when you have a possible Pituitary and/or

Hypothalamus

condition you need an MRi scan of the Head as well as various blood tests.

Fatigue can be

caused by many different hormones or glands going wrong such as Adrenal gland,

Pituitary,

Thyroid, Parathyroid and even Pancreas (Diabetes / Metabolic disorder etc).

I wanted to write as you have low Haemoglobin and Haemocrit levels.

Do ask for a Vitamin B12 and a Folate blood test as these relate to blood and

taking oxygen

to muscles and low or deficient levels caused exhaustion as well.

Additionally, Vitamin B12

deficiency can also be caused from Hypothyroid problems.

I briefly read where you question the Chlomid Test. All of these come under

" further

investigations " of the Pituitary and Hypothalamus and so try and be patient

whilst you

address the full picture rather than assuming all is caused from just one

problem at this

stage.

The endo probably becuase the pituitary controls the makings of some harmones in

the body

Yes but I don't feel it would be an Endo any Dr. that knows about pituitary

problems I would think it would be a brain Dr.can't think of there name. This

I only see one guy post that he seen an Endo that was good on Hormones. In all

the 27 yrs I have been on TRT and seen 7 Endo's not one got it right I am not

Primary but I am Secondary Hypopituitary and find Endo's are the last kind of

Dr. one needs to see for this problem.

This is my point about Endo's and Dr.'s not up on this. I went 23 seeing them

kind of Dr.'s and all that time they told me my Labs don't show me I am

Secondary they all told me I am Primary. I have lost count of how much money I

have wasted on them. After doing HCG and my Total Testosterone levels doubling

my new Dr. sent all my lab from yrs. back to a specialist this guy said my labs

were screaming a pituitary problem. The dam Dr.'s if they don't know should say

so not let a person go through out life because the did not want to admit they

don't know. My Prolactin levels were never high yet my pituitary dose not work

right. Your pituitary dose a lot more then tell your body to make more

Prolactin.

This is my point about Endo's and Dr.'s not up on this. I went 23 seeing them

kind of Dr.'s and all that time they told me my Labs don't show me I am

Secondary they all told me I am Primary. I have lost count of how much money I

have wasted on them. After doing HCG and my Total Testosterone levels doubling

my new Dr. sent all my lab from yrs. back to a specialist this guy said my labs

were screaming a pituitary problem. The dam Dr.'s if they don't know should say

so not let a person go through out life because the did not want to admit they

don't know. My Prolactin levels were never high yet my pituitary dose not work

right. Your pituitary dose a lot more then tell your body to make more

Prolactin.

Yes they are good Dr.'s for this problem as long as the DO is not an Endo Do

they just are not good Dr.'s. Before you see one call and ask questions like

you have been doing posting around the web. Ask if he dose the ACTH Stim test

to find out if one is Primary or Secondary. Ask how many men does he treat for

low testosterone and dose he test for high levels of Estradiol and if one is

high dose he use arimidex. Ask if a man is Secondary does he use HCG. You will

know if you have found a good one.

isnt it that a fairly elevated Prolactin level indicates the possibility of a

Pituitary TUmor? My level was just slightly elevated, but both my primary and

my regular endo said it WAS due to other meds I was taking that elevate

Prolactin slightly.

A lot of men that have higer levels of Prolactin should have an MRI on there

Pituitary if there tests show there Total T, LH, FSH low and there Estradiol

levels at about 20 pg/ml. To rule out a tumor on there pituitary.

Why is it that some guys in my school are always horny, but fat or really skinny

and weak? They have high testosterone, but low muscle tone and strength,

thinking about this I am still much stronger and thought of still as one of the

more muscular boys even though no one knows my problem. Is this just genetics or

what?

It's weird that they have the sex drive but are weak..

A lot of men that have higer levels of Prolactin should have an MRI on there

Pituitary if there tests show there Total T, LH, FSH low and there Estradiol

levels at about 20 pg/ml. To rule out a tumor on there pituitary.

Matt your going to be fine you did a good thing for your self you know something

is wrong and your going at with both guns. You will get to the bottom of all

this if whats wrong was your diet in time your body should fix it's self. Try

not to worry about this all the time the stress of worry will make it worse on

your body. I have talked to a lot of younger men into bodybuilding that messed

them self's up dieting to get there % of body fat down they end up with Adrenal

Fatigue and low Testosterone. They go back to a good diet and stop working out

until there Adrenals heal and they are doing fine today.

I think this should be done for two reasons, 1. it would determine if I'm

hypopituitary and if my testes still work, 2. It will determine my sensitivity

to hormones.

Dynamic Tests

GnRH Stimulation Test.

test (11,12), intravenous injection of 100

causes serum LH levels to increase threefold to sixfold

during a period of 30 to 45 minutes and FSH levels to

increase between 20 and 50%. Various degrees of primary

testicular failure cause higher than expected peak values

for LH and FSH. Men with hypothalamic or pituitary disease

may have a reduced or normal response that is often

inadequate for distinguishing between a pituitary and a

hypothalamic disorder. If the pituitary gland is primed

with repeated doses of GnRH, this stimulation test may

provide a more sensitive and reliable result.—In the GnRH stimulationμg of

GnRHClomiphene Stimulation Test.

stimulation test, 100 mg of clomiphene citrate is given for

5 to 7 days as an evocative test of the hypothalamic-pituitary

axis. Clomiphene acts by interrupting the negative

feedback loop and thereby stimulating release of

gonadotropin from the pituitary. A doubling of LH and a

20 to 50% increase in FSH are normal results indicative of

an intact hypothalamic-pituitary response (13).—In the clomiphene

Matt you need to know something about labs just because one is in the normal

range does not mean they are OK. Take a case of one guy that posted here he

seen an Endo because his Dr. found his testosterone was low 300 the Endo told

him he was fine because he was in the normal range. Yes he was but normal for a

man 100 yrs old. So what I am telling you if you have a low body temp with mid

range or lower labs, Free T4 and T3 and a less then 2 TSH you still can have a

low thyroid. Most good Dr.'s go by other symptoms to tell not just labs.

Yes you need to do this test and if you fale it do an ACTH stim. test also.

To see if your pituitary is sending ACTH to your adrenals to make enough

Cortisol.

With a lower Free T3 would like to see Total T4 and T3.

differentiate CNS from peripheral hypothyroidism), would it be desirable, e.g.,

adding pure T4?

T3 and T4 are bound to three proteins in the blood:

Thyroid Binding Globulin (also called Thyroxine-Binding Globulin, TBG)

Transthyretin

Albumin

The distribution of thyroid hormones and binding proteins are approximately:

T4: 68% to TBG, 11 % to Transthyretin, 20 % to Albumin

T3: 80 % to TBG, 9 % to Transthyretin, 11 % to Albumin

T4 has a stronger bond to TBG.

T3 has a stronger bond to Transthyretin

Both T3 and T4 have a much weaker bond (approximately 100 to 1000 x less) to

Albumin.

The thyroid hormones dynamically change between the free state and the bound

state. Since the bond to Albumin is weaker, much of what is bound to Albumin may

be free at any given moment, but won't be registered as Free T3 or Free T4. Some

portion of T3 and T4 is also free at any given moment but may not be registered

as Free T3 or Free T4.

Free T3 and Free T4 give you only a snapshot - one moment in time - of the

state. But this state varies from moment to moment.

This is why it is useful to take Total T3 and Total T4 into account to help

determine total thyroid function.

This is analogous to Testosterone. Some use " bioavailable " testosterone as a

measure of testosterone signaling activity. This would represent testosterone

that is free and testosterone that is loosely bound to albumin. However, even

tightly bound testosterone to sex-hormone binding globulin (SHBG) has signaling

functions via induced conformational changes in the SHBG molecule then binding

of testosterone-bound SHBG to SHBG receptors.

Thyroid hormone does not directly diffuse into cells.

Thyroid hormone is transported across cell membranes by various transporter

molecules. In the brain and in the blood brain barrier (BBB), two known

transporter molecules are Thyroid Hormone Transporter Molecule MCT8 and Organic

Anion Transporting Polypeptide OATP1C1. MCT8 is also produced in heart, kidney,

liver, and skeletal muscle.

There are two blood brain barriers: The Blood Brain Barrier Endothelial cells

that line the blood vessels of the brain and are connected to astrocytes of the

brain, and the Blood Cerebral Spinal Fluid Barrier Choroid Plexus Epithelial

Cells that connect the blood to the Cerebopinal Fluid. The Choroid Plexus

filters blood in order to produce Cerebrospinal Fluid.

From blood, T3 and T4 enter the brain via two paths:

1. T3 and T4 are transported into a BBB Endothelium Cell (via OATP). T3 and T4

are then transferred into an attached Astrocyte. In the Astrocyte, Deiodinase D2

coverts T4 to T3. T3 then exits the Astrocyte via MCT8. T3 then enters neurons

via MCT8 transporters.

2. T3 and T4 are transported into Blood Cerebral Spinal Fluid Barrier Choroid

Plexus Epithelial Cells (via MCT8). They they exit the choroid plexus (via OATP)

and enter the Cerebrospinal Fluid (CSF). From the CSF, T3 and T4 are taken up by

Tanycytes or Astrocytes. These cells have D2 Diodinase, which convert T4 to T3.

Upon exiting these cells, T3 enters neurons.

Notably, neurons have Diodinase D3 enzyme which converts T4 to reverse T3 and T3

to T2.

The presence of thyroid hormone can reduce production of OATP as part of a

negative feedback loop control.

Within brain cells, there are variations nuclear membrane thyroid transporters.

Usually, 90 % of the intracellular T3 is located in the cytosol and 10 % is in

the nucleus. In the pituitary gland's cells, however, 50 % of T3 is in the

nucleus.

Adding to the complexity of how thyroid hormone works, there is an Intracellular

T3 Binding Protein (CTPB) which is produced in high amounts in the brain and

heart, though is also widespread in production in the body.

Serum measurements of thyroid hormone can't be used to infer CSF concentrations.

Only a spinal tap will be able to tell what the CSF concentrations are.

Brain thyroid hormone levels and T3 to T4 ratios are going to be determined at

several levels.

For example, the number and types and location of thyroid transporters

determines what amount of thyroid hormone gets through.

Variations in the genes for the thyroid transporter molecules will determine how

effective they are and how selective they are for T3 or T4 transport - creating

a difference between Blood and Brain concentrations of Thyroid hormones.

Variations in Astrocyte and Tanycyte Diodinase D2 production will determine T3

to T4 conversions in the brain, which may be different from the blood.

From my point of view, given the differences that can arise in blood versus

brain levels of thyroid hormone and thyroid hormone conversion, it is important

to consider in some patients to not only optimize T3 but to also optimize T4

levels.

This is important, for example, in mood disorders. Here, the difference between

T3 and T4 treatment becomes apparent.

In major depressive disorder, historically, T3 is a more effective treatment

than T4 in reducing depressive symptoms. Spectulating: perhaps T4 to T3

conversion in the brain's astrocytes and tanycytes is impaired by lack of D2

Diodinase production, among other possible problems in brain thyroid hormone

metabolism.

In bipolar disorder, historically, T4 is much more effective than T3 in

stabilizing mood. T4 may be used medicinally to reach " hyperthyroid " levels -

based on TSH measurements - in psychiatry to stabilize mood in bipolar disorder.

Speculating: perhaps, in bipolar disorder, there is a gene mutation in one of

the thyroid transport molecules which selectively impairs T3 transport.

If a person is having problems with a T3 treatment or Armour Thyroid Treatment

(which is primarily a T3 treatment), then perhaps adding a T4 treatment would be

useful. Some patients benefit from combinations of thyroid treatments (e.g. T3 +

T4, Armour Thyroid + Levothyroxine) better than single treatments alone.

I've had a 3x a day cortisol saliva test each one coming back normal.

Yes you need to do this test and if you fale it do an ACTH stim. test also.

To see if your pituitary is sending ACTH to your adrenals to make enough

Cortisol.

Please post the test with the units and ranges as you can see in the link I sent

you there are stages of this the first stage is when your high across that day.

Later on in the end stages your at the bottom and DHTH works in lock step with

this.

See your morning cortisol levels if your adrenals were working normal would be

up onto the upper end of the morning range your dam low for a morning cortisol.

Most people with healthy adreanls have levels up near the top of the range for

the morning test. Your dam low problem with Dr.'s is if your in the range your

OK this is not true. Dr.'s not knowing about Adreanls and there problems only

look for 's or Cushing's problems this is like black or white no in

between. You look to me to be at or near stage 4 did you have DHEA tested if

so was it low this is a clue to having this problem.

Notice the low morning levels of Cortisol. These people have a hard time

" getting going " in the morning. They become night and afternoon people. With the

high night-time Cortisol They often have trouble getting to sleep and may awaken

when their blood sugar levels drop (about 3 to 5 am).

Notice that there is very little change in the Cortisol levels. This is a

certain sign that the body is in real trouble. We see many patients with this

type of reading.

Why can't doctor's say this to men when they are going through this issue? I

know that male pride is delicate but it doesn't help to tell someone they are ok

when they are in a erry low range of " normal " .

Most of them don't now enough about this topic to understand what
