To me ,a health care provider,a doctor,what is important is every single life. I fail to understand ( and I am sure I am not the only one in the crowd) where is human angle gone in treating patients. I totally agree with Umesh,rules and guidelines are OK,but they should not be in lieu of some one's life. Not only that, I reiterate my earlier stand that where was govt/NACO,as far as HIV treatment from outside ART is concerned in the begining epidemic. Why should PLHA be punished for trying to survive in those times with the help of doctors who had a passion for these unfortunate victims?. Why is NACO silent about ever changing guidelines all over the world---even they are changing (eg.cut off for asymptomatic patient is 250 now and not 200 as was the case not very long ago in NACO controlled ART centers). These medical officers are the one in the field and know the patient individually rather than the team located somewhere else. Afterall this secondline do need a trainig of these art medical officers and strategy of testing all the patients for their ontreatment viral load status am sure as the art program got its success this second line program would never be a waste of money as all our art centres have proven their efficacy nation wide and now its time to think of decentralising this secondline art program. all our medical officers would never waste a single penny of the government when compared to all the scams around the nation, this essential step would serve a noble cause a strategical training and proper use of the manpower resources of our national program would give better results.
