Guest guest Posted November 10, 2010 Report Share Posted November 10, 2010 Hi cjsheets24 and Welcome, Your Dr. needed to test you 2 weeks after starting you on this low starting dose of Androgel. You can end up lower then you started if this dose it to low of it does not get through your skin. Most men do better on 10 grams of Andorgel You need to spread this over your upper arms and shoulders and down your back as far as you can reach after a shower but let it dry from wet just spread out around real good. Then do the other side here is a copy of a file I made about using gels. If you try the higher 10 gram dose and your levels are still to low it's your skin and you need to do shots. ==================================================== Starting on Androgel or Testim. Yes this happens a lot you start on a low starting dose 5 grams when your brain sees the testosterone in your blood even just a little it slows down even stops sending the LH and FSH message to your testis to make the Testosterone you were making. So lets say your labs for Total T were 350 you add this dose of gel your brain sees this and you lose the base level you had before the gel. Now lets say you need more then 5 grams most do so now all your left with is what the gel is doing so lets say it's doing 200 so you lost 150. The reason you feel good is your levels go up that 200 the gel is doing in the first 2 weeks so your levels go up to 550 in theory. The brain sees this and you fall back. Why can this happen it might be how your putting on the gel with Androgel you need to spread it over your upper arms and shoulders and down your back as far as you can reach this is half of your dose then do the other side. Just spread it over the area but don't rub it in. The other thing is it's not getting through your skin good so you need more. If you have a thyroid problem your skin will become thicker and gels and creams don't get through the skin and you need to switch to shots. I tell men when they go on Gels to retest in 2 weeks because you can end up feeling worse. Most men just need more gel. Dr.'s that tell men to go on 5 grams of gel and come back in 8 to 12 weeks for labs don't know much about the use of gels. Dr.'s that treat a lot of men for low T on gels see this happen and test them again in 2 weeks. When you go to do labs get up and shower that morning go do your labs and then put in the gel after your blood test so you don’t spike up your labs. If you can’t do labs in the morning shower put on the gel but don’t get any gel on the spot they take the blood from. Do your labs about 6 hrs later. Call your Dr. and tell him this and that your not feeling good you were but fell back. I remember my doctor telling me 5 months ago that the gel absorbes better through the stomach so I have been doing it that way. I remember feeling like a new man in regards to my labido when I first started taking it. Then the dreaded back down cycle. I will start taking the 5 grams the way you suggested. On a side note. I am very concerned as I just lost my job and now don't have insurance and can't afford cobra. I have 2.5 bottles of Androgel left. I want to do the therapy correctly and continue on it if it's going to help me get my life back. My girlfriend has been very patient with my issue, but my lack of sex drive is really weighing on her. Do you know any assistance programs as long as my doctor keeps prescribing it? Also, I forgot to note I am 37 years old. Doing your own shots 100 mgs per week for about $120 to $150 that will last you about 19 weeks. Here is a link about doing shots read the info page and have your Dr. show you how to do them. http://spotinjections.com/index3.htm I use a 27g 1ml X 1/2 " needle and shoot into my thigh no pain this way and I have dam good levels. The oil comes out slow so I just pull the plunger all the way down and hold it until I have my dose takes about a min. or two. I tried to get my doctor(who is about to retire) to give me a prescription for injections but he said that there would be several blood tests that would have to be done and with my lack of insurance it would be expensive so I just forgot about it. I can not forget about it anymore. It's affecting my life negatively in all aspects. I am depressed, have absolutely no energy and drive to do anything. It's time to take this serious and find out what's wrong with me. I'm ready to get all the necessary bloodwork done to see WHY my body is failing me and get on a treatment plan. My mom has been on synthroid for years and my dad has low-t has well so I have some hereditary things probably going on too. My girlfriend suggested going to BodyLogicMD. They have a local office here in Columbus, Ohio. http://www.bodylogicmd.com/ She a believer in the bioidential hormone thing. Do any off you know anything about them or think that is a good place to start? It's $595 for the bloodwork through Quest and then $395 for the initial consultation. Should I call a local compounding pharmacy to see who's buying testosterone and then contact them based on the " Finding a Male Hormone Doctor " page? I would love to have all your expertise on what I should do to get my bloodwork done and find a knowledgeable person to help me feel better again. It's all going to be out of pocket due to not having insurance but my family is going to loams me all the monetary means I need to feel well again. I read Dr. Crisler's website and articles to understand things better about 2 months ago and reread them the other day. I talked to my girlfriend about it and she even read the articles because she's very into hormones and how they effect the body. She had problems for years and did research to find the right doctor too. If she can drive 2 hours for her doctor so can I. I signed up for Life Extension last week. I spent about hour and fifteen minutes with him and felt great about moving forward with someone who can help me. By the way he has this little chiuaua rescue dog named Tox. He is the cutest little thing. Ok back to topic. I did a 24 hour urine analysis and a complete blood test. The urine results wont be available until next Monday. Here are the blood results though. reference interval LH 1.7 mIU/ml 1.7-8.6 FSH 1.1 miU/ml 1.2-12.4 Test Total 333 ng/dl 249-836 Free Test 8.4 pg/ml 8.7-25.1 SHBG 27.7 nmol/L 14.5-48.4 Some other interesting test results: Glucose,serum 104 65-69 This is showing prediabetic. I cross referenced this with a symptom of hyperthyroidism which for some reason the doc didn't chose to initially check. Cholesterol, Total 248 100-199 I eat a pretty low fat diet and I'm 6'1 and about 185 so I don't carry alot of excess fat around so this might be something genetic as my dad is on cholesterol pills but he is obese and eats terribly. WBC 3.8 4-10.5 Third time in a year my WBC has been down. I'm patiently waiting to hear from Dr. Crisler on the course of action but it appears I have secondary hypogonadism. I have felt awful for years and I'm excited to know I am closer to figuring this out and getting my life back. Thank you guys for reading and listening. I was on TRT 23 yr being told I am Primary meaning my testis don't work. He sent me a post to help me get my Dr. to let me try HCG. Here is a copy of it. =================================================== He probably feels that since you suffer primary hypogonadism (I am guessing) there is no use in adding HCG to your protocol. There are several reasons why this is not so. First, you have not lost all Leydig cells, so any HCG you take will stimulate those who still function to produce endogenous testosterone. This will support testicular size. We should not ignore this aesthetic consideration. Next, if he reads my work, he will learn that HPTA-suppressed (as all TRT patients are to some extent) also suffer decreased pregenenolone levels, which is the first step after CHOL in all three hormonal pathways which begin with CHOL. HCG increases pregnenolone production, and therefore restores a more natural balance of our hormones. Next, nearly all TRT patients who add in HCG to their regimens report an increased sense of well-being and also libido. These are genuine quality of life issues. Finally, I just instinctively do not want all those LH receptors (including those we have yet to discover and appreciate) unstimulated. --------------------------------------------------- My Dr. read this put me on HCG after 15 shots we did labs and my TT levels went from 600 to 1200 so my testis work and I am Secondary but from a head injury due to an Auto Accident. I must tell you if Dr. did not tell you don't post what he is doing to treat you keep it general or if he sees you talking in detail about what he is doing to treat you he will get dam mad. What he dose for you is for you and might not work on others. If you post what he is doing for you others will try it. And if it dose not work they then go on the forums talking about him like a dog. If men are seeing a Dr. and want him to go over there labs and treatment he will do this over the phone or by Email. They don't even need to see him but it's not free. Test and hcg? hcg only? Clomid? Whatever it is I'm interested in hearing and what your results have been? Our bodies need enough of the right type of dietary fat for various reasons, including to make hormones. I figure your doctor will be able to help on that if you've got questions. I do know he likes to treat most men with TRT and HCG he starts out with compounded gels everyday and has his men do 250 IU's of HCG every 3 days. Yet I know some men that do HCG only one guy sees Dr. Shippen and dose 60 IU's of HCG everyday. And uses Indolplex/DIM to keep his Estradiol E2 levels down here are some links to his works. I ordered the scripts and I'm waiting anxiously to get started. He said I could try a restart with Clomid if I wanted to, but recommeneded TRT. What I am having trouble with is 24 hour urine test results hadn't come in yet and we still moved forward. I would have thought with me being secondary that those results with be very important. I am concerned about the reason I'm secondary. I am concerned about being potentially hyperthyroid (he didn't have thyroid tested) or having adrenal issues. Could I have pituitary tumor? I was so excited to be going to a doctor who was at the forefront of these issues and I feel letdown that more expertise wasn't used. I am very concerned. Am I over-reacting or should I have a right to be? He is my doctor, but who am I to question him? Help!!! So lets say it this way your Dr. did labs your Total Testosterone came back very low if he also tested your LH an FSH and this was low it looks like your Secondary but Estraidol can do this to labs so you need this tested also. Here is what happens when Estraidol levels are very high it will look to your brain like it's high Testosterone because Estraidol is made from Testosterone and your brain can't tell the diff. between high Estraidol and high Testosterone. So seeing high levels of Estradiol your brain will think it's Testosterone and slow down sending LH and FSH so your labs look like your Secondary. All that is needed when this happens is to take a med to lower your Estradiol when it comes down to about 20 pg/ml redo your labs your TT levels can come back up and your LH and FSH levels with it. If not then you are Secondary and need an MRI on you Pituitary to rule out a tumor. Here is a link about this but in older men I still see this happen to younger men. Guest guest Posted March 11, 2011 Report Share Posted March 11, 2011 Charlie, Your labs look like your Secondary but without the Estradiol test if your Estradiol levels are high labs can look like your Secondary. My Dr. will not do 24 hr urine labs for hormones he says they only show what your body is dumping and they are Free's. Even my BCBS will not cover the cost for them and I see a lot of them labs at Dr. 's forum and not being a Dr. I can't tell what they mean. Hard tells me they are good labs when your on TRT they show what is going on. But you need a dam good Hormone Dr. that knows how to read them. Lets say your Cortisol levels come back high at the top of the range. This says nothing about what your body is doing Cortisol levels start out very high first thing in the morning. Going down by bedtime if your Urine labs look high or OK you can still have a problem. Doing a 4x's in a day Saliva test show what your rhythm is doing you might be very high at night when you need to be lower. Higher levels at night keep you from sleeping good. Here is a link about how the Testosterone in your body works and most good Dr.'s dose your T meds by blood labs testing Total Testosterone you need good levels for all this to work. I don't feel you have high Estradiol levels going by your SHBG levels at 27 but you still need to check it. I did an MRI on my Pituitary it came back clean. Yet my Pituitary is bad dose not work right due to a head injury from an Auto Accident some 30 yrs ago. It took me 23 yrs to figure all this out being on TRT all them yrs not feeling 100%. Dr.'s look at my labs seeing them in the low normal rage telling me I am fine. This is a big problem with Dr.'s when someone has a mess of labs in the low normal range it's a Red Flag going up. So when we figured out I am Hypopituitary and started treating my low Cortisol, Thyroid, Ferritin Iron, Aldosterone and plasma renin levels I started feeling better. Even my Growth Horomens were low but I can't take GH meds I hold water big time I can't feel things with my hands and feet. Guest guest Posted March 14, 2011 Report Share Posted March 14, 2011 I got my urine labs results today and it's not good. He said on a 45-85 scale for testosterone I'm 30. Also my DHEA levels are as low as he has ever seen at 4.7 And the kicker is it looks like I have adrenal fatigue. I am starting to get overwhelmed with all the different issues I'm having. Anybody has good experience with DHEA? I don't know if I will go with a compounded transdermal or over the counter supplements.
