His Dr. is not up on give out TRT he might know about it but what he is doing is old out dated. Giving him shots 100 mgs every 15 days he is driving down his levels even lower. What happens when a man adds Testosterone to his body his brain sees this and slows down sending the LH and FSH message to his testis to make more Testosterone. Most men with low levels of Testosterone sill feel like having sex I was at 120 for Total Testosterone and age 37 I still had sex with my wife 2 to 3 x's / week. But I needed to make it a point to do so the fatigue and feelings of Depression were so bad I did not feel like having sex. Men today start on shots doing them 100 mgs / week and we add in HCG (Human Chorionic Gonadotropin)250 IU's the 2 days each before his next Test C shot. We do our own shots at home some men need to do them 2x's / weeks 50 to 80 mgs to keep leveled. Also a big killer of a mans libido is high levels of Estradiol E2 have this tested do a Quest labs Sensitive Estradiol test #4021 the range is 13 to 54 pg/ml best levels for men are 20 if his labs are over 30 he needs to take something to bring his E2 levels down. One way to tell if his E2 is very high is he will not have night time and morning wood in his sleep. A typical recommended dose would be 100 mg every week or 200 mg every two weeks. Some doctors even prescribe 400 mg once a month, although that amounts to quite a roller coaster. Basically he is getting " half " the normal dose, but I suspect the doctor is trying that out and may be persuaded to go to a full dose if it shows promising results. Starting out with a full dose of a new treatment could be considered inappropriate. It takes some time for the body to re-adjust after starting. I know it took me about 3 months to adjust and really feel good. After being hypogonadic for years it takes a while before the body's cells realize that the testosterone is available and to start using it. Testosterone works and the cellular level and activates a gene that controls the action of mitochrondria, the " Powerhouses " of the cell. After 3 years of testosterone replacement at 100 mg every week, I cut back to 80 mg every week. Testosterone Cypionate has a half-life of 8 days. It also peaks on the third day after the injection. You need to realize that half life of a shot means that there is still some residual many weeks later from any given shot. 8 days 1/2 16 days 1/4 24 days 1/8 32 days 1/16 40 days 1/32 Lets look at what happens at day 16. The old shot has a level of 1/4 when he gets the new shot. 8 days later the second shot is at 1/2 level, but you have 1/8 from the first shot. Have you ever heard of Zeno's paradox? Google it up sometime. He said that if you were a distance from a wall and took a series of steps " halfway to the wall " you would never ever get there. You would soon get so close it didn't matter, as long as you went half the remaining distance you would never get there. Similarly every 8 days the remaining T decreases but never goes to zero. It would take an excel spreadsheet (or a calculus course)to show how shot after shot builds up in your system, but it does. Yes, more frequent shots does lead to a more constant level of testosterone. I question how necessary that is though. Since the blood level of T doesn't peak until day 3, doing shots every other day seems excessive to me. You are adding in more before reaching the peak. Guest guest Posted March 18, 2011 Report Share Posted March 18, 2011 DH does 25 mg every 5 days, so in 45 days, he has taken 225 mg. That is far less than the 300 mgs you're recommending. There are several points to be made: 1. Dosing is VERY individual. I have seen guys do 20-100 mg per shot. 2. How often you must dose is very dependent on your SHBG levels, which is often dependent on your thyroid levels. Low thyroid means low SHBG, so testosterone does not last, and you have to dose more often. 3. reported " He is always so tired, depressed and irritable. " These are common hypothyroid symptoms. If he really is low in thyroid, all the testosterone in the world isn't going to fix that problem. That needs to get checked out. " I personally think weekly shots are just right and that's what I use. Many insurance companies won't pay for a doctor's visit more often than 2 weeks. Guest guest Posted March 19, 2011 Report Share Posted March 19, 2011 I think you have it about right.I once took a shot every 84 hrs (3 1/2 days).Now have switched to once a week.Actually feel better and have less sore muscles from injections.I have taken up some weight lifting, moderate amy for a 55 yo man.I am amazed with the quick results I have obtained.Sexually i am good to go.Can't get much better than that. I know I do if my levels gets that low. However I had a failing testicle and never really noticed until it was time to have it out and I got tested and it came back at 271, I felt ok, functioned sexually but lacked a lot of energy. Ok, people are saying go higher dosage or more often or whatever. Yeah, ok, but be prepared for a roller coaster ride (I am 32 if that matters). The papers will say that guys in the early 30s should have a T level of like 800 on average, if I have a T level of 800 I have rage issues which then turns to depression. I quit taking 250mgs every 2 weeks for that reason an the roller coaster that is what I would refer to as a male version of PMS. Going from high energy and raging libido down and crashing after about 8 days is hell. My wife and son could not live with it, thus I am on 125mgs every week. Not great but not terrible. I still have a healthy libido, can sleep and wake up. Consider all sides of the issue before jumping to add more hormones. Japan is a different country with different systems than you have in the US, there are no especially good endocrinologists in my area and I am working with a urologist at the moment. I am no double sure that the gel is the best option as it has a 24 hour cycle. I can't get the gel here. I am also sure that 50mgs every 72 hours is a really nice way to stay ahead of any downturn in levels again not an option for me. What is the HCG though? Perhaps I should add that both of my testicles have been surgically removed, thus I am working with the urologist who did the last surgery. Guest guest Posted March 21, 2011 Report Share Posted March 21, 2011 Before condeming the guy to TRT one needs to look at 1. Why is his testosterone levels are low? 2. How old is he? If his dr puts him on TRT at age <25 then he is a complete idiot with out looking for reason why. TRT in these cases are a bandaid. I tend to look for factors that affect testosterone levels then address them at the source of the problem them trying to use bandaid methods such as TRT (only as last resort) There are just to many unknown variables to contend with by handing this through a forum setting.
