Guest guest
Thanks for the input. I appreciate all avenues of thought and will have everything checked out. I am a little uncomfortable giving seizure medicine unless they find seizures, but at the same time we have a violent 17 year old who can easily put me away and hold his own with his Dad. It seems that he is getting some kind of satisfaction out of pulling us into this power struggle to keep him from hurting himself. I noticed yesterday him going to the mirror to see how much damage he had done to his head. We are a mild mannering peaceful family. We don't understand this and are at a loss for a solution.  Thanks again. You gave me some more avenues to look at.  Drissia From: Popplewell <JPopplewell@ vmrc.net<mailto: JPopplewell %40vmrc.net> > Subject: RE: [ ] Re: Help " <mailto:Autism- Mercury%40g roups.com> " < <mailto:Autism- Mercury%40g roups.com> > Date: Thursday, May 6, 2010, 3:34 PM This doesn't sound to me like a clear-cut case of PANDAS. As initially defined, PANDAS should begin before the onset of puberty and I expect a 17 year old has passed puberty. Additionally, PANDAS includes tics or obsessive-compulsiv e behavior; head-banging may fall under this heading but seems a little extreme. Of more concern is the suggestion of autonomic involvement reflected in the pallor and episodic blue lips. While there is probably little harm involved in taking penicillin until age 21, I would recommend against stopping the search for other causes. Guest guest
Multiply the 1000 ml times 15 drop/ml (15,000). Divide that total by your 8 hours multiplied by 60 minutes (480 minutes). 15000 divided by 480 = 31.25 drops per minute. I hope this helps. Guest guest
Well Done Neal and Crystal! The original poster has thanked you! I am concerned now with others who may want a bit more or a faster way. Or after you have mastered the what you have seen on this site posting you may want to change to this method or thinking process: I am going to show another way. I call it 'string math' and I coined that term. No other book or author has or can use it. Technically it is dimensional analysis. The problem should read: How many drops per minute will a patient receive if an IV of 1000 ml if 5% Dextrose Infusion is to run in over eight hours? The drip factor is 15 drop/ml The formula or how to 'think' in solving the problem is: Drop Factor x ml/hr x Time Conversion (hr conversion) or simply : DF x IV Rate x TC or DF x IV R x TC 15 gtts -----1000 ml --------1 hr ------ 15000 gtts -------- x ----------- x --------- = ----------- = 31.25 gtt/min 1ml-----------8 hrs----------60 min-------480 min If the above does not come out well in the posting due to HTML, I am also placing the same in a diagonal way or using a 'slash ' instead of a horizontal dash for a fraction line. 15 gtts/1ml x 1000 ml/8 hrs x 1 hr/60min IF this looks too difficult then 'piece meal it' in baby steps: DF x IV Rate x TC Drop Factor TIMES 15 gtts/1ml Drop Factor TIMES IV Rate TIMES 15 gtts/1ml X 1000 ml/8 hrs X Drop Factor TIMES IV Rate TIMES TC 15 gtts/1ml X 1000 ml/8 hrs X 1 hr/60min What ever method floats your boat! Which ever you are more comfortable with go for it! In the beginning I think the methods outlined by Crystal and Neal (as well as in my tutorials) are best. This may use ratio/proportion and gets you to think about each piece. As you get more experienced doing these problems the dimensional analysis may be a good method for you as it is quick and you are now familiar with the 'parts' of the set up. You recognize each part: the rate ml/hr , the drop factor in gtts/ml and you already know how to convert or the Time Conversion of 60 min/hr Best of Luck to you all, Jeanetta Mastron CPhT BS Pharm Tech Educator Founder/Owner > > How many drops per minute will a patient receive if and IV of 1000 ml if 5% > Dextrose Injection is run in over eigth hours? Guest guest
Karin, There is the 's Rule which applies here, not used much any more but can be applied. The BSA is usually used. Teh formula is a way for a doctor nurse or pharmacist to 'guage' what a child's dose could or should be based upon an adults dose. So if 100 mg is given to an adult, a child of 14 lbs may be given oly 9.33 mg based upon the 's Rule/formula. Now of course that is ONLY if a child should be given the drug. Some drugs are contraindicated in children. So it should then be more like: Using 's Rule, a drug is not contraindicated in children and given as 100 mg to an adult, would be dosed as 9.33 mg for a 14 lb child. Child's Wt in Lbs X Adult Dose ------------------------------ = Child's dose 150 lbs 14 lbs x 100 mg ---------------- = Child's Dose = 9.33mg 150 lbs Not used much, but a good exercise in calculations. So I guess this is why PTCB still uses this formula. At least it is still in the study books. Any newbies know for sure if this formula is on a recent PTCB exam? Not asking if the problem is, but is the topic of 's Rule on the exam? Pediatric Dosing? Guest guest
I can't help you with the implant issues but until you figure that part out you did not mention trying antihistamines for the hives before being put on the steroids. Some folks respond to antihistamines (even metal allergies) while trying to figure out what is causing their allergy and others do not. I had two years of chronic (constant) urticaria (not from any orthopedic implant or ortho related product) which caused me to have to take a 24 hr non sleepy antihistamine every day or keep a certain level in mys system daily. I think I got it down to where 1/2 of an RX size dose would work daily or a full dose every other day but I had to keep it in my system constantly to ward off the hives which were popping up in no special place but anywhere and pretty much everywhere I was not wanting them. To this day so far whenever I get a few hrs worth of acute hives from one or another of my known causes of hives I seem to be able to take any of the antihistamines (4 hr sleepy causing types all the way to the 24 hrs non sleepy types) depending on how long I anticipate the attack to last and/or if the itching is threatening to cause me infection or severely upsetting my social or work life at the time. Maybe you should go back to your primary to see what allergy testing she might suggest and where you could go to get tested. Guest guest
Jen,At this point I am concerned about your doctors way of doing things. Is he a gastro or a liver specialist or just a GP? The Interfuron does not cause the anemia.. the Ribavirin does. He should know this. A half dose of Interfuron isn't going to do much to kick the Hep C. And 1200 mg's of Ribavirin a day is within the recommended dosage but you could go lower on that and still have the benefit of the drug but without the Interfuron at a full dosage I'm tending to think you might be spinning your wheels. This is the point where I personally would exit stage left and find another doctor post haste! Pro-Crit is a weekly shot that you take that helps stimulate red blood cells. The other option is a blood transfusion but only if you are very anemic. My doctor made sure that I was not anemic prior to treatment. I had been anemic my whole life and that wasn't going to fly with him during tx. I had female procedures done to fix that issue and had to be in good shape blood wise to start tx. My blood counts started tanking about my 4th week into treatment. I had my first blood transfusion at week 6. I was taking more than double the amount of Ribavirin you are taking and the maximum dosage of Interfuron. There are, in fact, some other options out there for Hep C, however, I have not heard of anything that will rid that virus from your system other than this treatment. Dr. Berkenson has a treatment protocol out there that I believe consists mainly of diet changes. SuziQ is following that and has kept her Hep C at bay for many many years. His protocol is listed in the links library. I would like to know what your doctor is calling critically low. If your blood counts are, in fact, critically low, you would know it. You would be short of breath, extremely tired and able to feel your heart beating when you walk across a room. The insides of your eyes will also be a very different color from the normal pink to red that they are... Go to a mirror and look at the inside of your eyelids... actually pull your bottom eyelid out just a bit so you can see the color there.. If they are dark pink to almost red, you're okay.. if they are almost white, you're critically anemic. Someone who is critically anemic needs a blood transfusion. My guess is that your blood count is showing some signs of compromise and he is panicking. Your blood count is going to drop during treatment, but this doctor has me wondering... His initial idea is just not making sense... the dosage that would need to be adjusted would be the Ribavirin and there comes a point when if you adjust that too low it's not able to work. My thinking at this point would be a new doctor. See if you can find a Hepatologist... A Liver Disease Specialist. They know how to play the Hep C game better than any doctors out there.. if your city or town doesn't have one, your next best bet is a Gastroenterologist. I would not, under any circumstances let a run of the mill Family Practice guy orchestrate this treatment. Hang in there.. the world is not ending. I promise you that much. I will be more than happy to help you figure this out. Hugs, TeriOn Thu, Oct 21, 2010 at 6:28 PM, Crouch <lucky2b916@...> wrote: I just got a message from my doctor's office. Guest guest
Hey back to you both: Thanks for the input! I had taken a brief look at Dr. Berkson's eating suggestions the other evening but haven't had time to delve into it. I will follow up when I have more time this weekend and I may want additional information because I want to fight this on as many fronts as I can. I talked with my trainer yesterday and he is prepared to help me exercise-wise and diet wise (to the degree he can on that front) because it is a challenge for him to keep me healthy. My doctor is a gastroenterologist (sp?) and has experience with treating Hep C patients ... but I don't know how many or for how lon. Also, just read that immunizations and other things like should be discussed with the doc before doing them. I took a flu shot last week without having a thought about doing it .... could that have impacted my blood counts? My eyes seem to be pink to red and I'm not overly fatigued at all. I plan to ask what my counts are and about the Ribavirin dosage when I go in. I do think he is being overly reactive. I will also ask him about procrit and transfusions. The blood doc I went to back in July said he would recommend I continue treatment and monitor the blood counts, so I am taken aback by this recommendation, again, particularly when I don't feel bad. Thank you both for your help. Big hugs, Jen > > > > > > I just got a message from my doctor's office. Guest guest
I am really concerned about the half dosage of Interfuron.. Not sure its enough to do the deed. If your eye lids look okay, you're probably fine.. Mine turned white! I had two transfusions and then went on Pro-Crit for several months... My treatment was never even compromised. Go in there with questions that are on point. What are my numbers? If your Hemoglobin is over 9, you are FINE. Why the decrease in the Interfuron and not the Ribavirin? Let him know that you know it's the Ribavirin that destroys the blood cells. The flu shot wouldn't affect treatment.. I was made to have a flu shot and a pneumonia shot on tx. I took a flu shot last week without having a thought about doing it ... could that have impacted my blood counts? My eyes seem to be pink to red and I'm not overly fatigued at all. I plan to ask what my counts are and about the Ribavirin dosage when I go in. I do think he is being overly reactive. I will also ask him about procrit and transfusions. The blood doc I went to back in July said he would recommend I continue treatment and monitor the blood counts, so I am taken aback by this recommendation, again, particularly when I don't feel bad. Thank you both for your help. Big hugs, Jen > > > > > > I just got a message from my doctor's office. Guest guest
TrudyI don't recommend what I did with the Riba during my first tx; but, by the end of the 48 weeks, I had a full box of Riba left. However, I also had HepC still, as well. Nobody believes that missing the Riba's had anything to do with the virus mutating though, since it only happened late in my course. Is there anyway you can count your Riba that is left and see if you come up with two extra - or is it three? Anyway, that's what I would do. Don't lose any sleep over last nights pills and give yourself some time before drinking more water that is going to make you throw up.Please don't worry, you are showing that you kicked the butt of the dragon. Now, you are in count down mode and that's when I started to just get tired of swallowing pills.Gloria Hey guys, last night I believe I threw up my Riba. This morning I was taking meds & got distracted.( pain issues). I am pretty sure I haven't taken this mornings Riba; take them now? Even toh it COULD be a double dose. Sometimes I throw up other meds, but never the Riba till last night. And even then , I'm not sure. http://facebook.com/people/andTrudy-Kinsey/1340460877 " "A well- behaved woman never made history"...Mae West http://oktravels.wordpress.com http://allrecipes.com/cook/TrudyK/profile.aspx
Guest guest
Trudy, I would not take it if your not sure. My doctor took me off the Riba's for 2 weeks when I had that rash and at my 16 week check up I was still undectable and Hep C negative. I would rather not take it then to double dose yourself on that nasty stuff. I have to set the alarm on my cell phone so I remember to take them, then I don't turn the alarm off until I have put the pills in my mouth. LOL Sad but I have a lot going on, it's hard to keep up with everything, Good luck

Guest guest
Call the doctor Trudy.... Not going to even try to tell you what to do on this one.. I know what my initial response would be but I'm not going to even open my mouth. Call the doctor and ask him what to do. Let me know what he says..Hugs,TeriOn Thu, Dec 16, 2010 at 11:25 AM, trudykinsey@... <trudykinsey@...> wrote: Hey guys, last night I believe I threw up my Riba. Guest guest
Now THAT is a really good idea! Set the alarm on your cell phone to remember to take your pills! Guest guest
I had a whole jug left over too..I have different ones, 600 mg tabs and they r not as harsh to my stomach. I saw the doc today and I still have a viral load...it's only 160 but man I was hoping 0. My rash is full blown, guess I'll be setting another record for cutanious outbreak..gotta be good at something! She gave me a lab slip for tomorrow and maybe it'll be 0 this time. All other bloodwork was good. Not dwelling on VL as we all know true test is when TX is over. Guest guest
What an excellent idea!! The only thing to remember, is if you go out, you'd better have the pills with you.I use the alarm on my cell to wake me up too. It's so annoying when I leave it downstairs because it doesn't quit until I manually get out of bed and come down and make it stop.Gloria Trudy, I would not take it if your not sure. Guest guest
PamThat's very good, not stressing about the VL. As I heard it once, worrying is like sitting on a rocking horse - lots of action; but, you're going nowhere. Please remind me what week you are in with your meds. Besides, I have to have a candle for you as well. I'm putting your name on one right now. Do you do your shots on a Friday night??Gloria I had a whole jug left over too..I have different ones, 600 mg tabs and they r not as harsh to my stomach.
