In men lets not call it Estrogen it's Estradiol E2 the bad guy here for men.

And yes I feel any Dr. finding a man with low Testosterone they should test his

Estradiol levels before putting him on TRT or Androgel. Here is why in older

men and some younger men there levels of E2 can become high and the brain sees

this not as E2 but as Testosterone because a man's E2 is made from Testosterone

and the brain can't tell the difference between them. So if a man's E2 levels

are very high over 30 pg/ml the brain will think this is Testosterone and slow

down sending the LH and FSH messages to the Testis that tell it to make

Testosterone and the poor guy ends up with lower levels of Testosterone.

If a Dr. would just test this and treat his E2 levels first a man's Total

Testosterone levels can go up some 200 to 300 points and he will not need

Androgel for life.

One more point a lot of Dr.'s see a man with low levels of Testosterone and just

put him on TRT with out looking deeper into why he is low. Not testing E2

levels a man can show low Testosterone levels with low LH and FSH and think he

is Secondary or his Pituitary is not working and order MRI's that cost a lot of

money. And all it is his high levels of E2 the labs look the same as a man with

a Pituitary Problem.

Ideally, a doctor should test estradiol (E2) upon presentation of a patient with

signs & symptoms of low testosterone, but alas, few test estradiol (at the

beginning, if at all). Even if it's determined that a man does not have high

estradiol causing low testosterone, should that man proceed with testosterone

replacement therapy, he should get an estradiol lab in conjunction with his

testosterone & CBC/CMP lab tests. It's even more urgent if his doctor (wrongly)

prescribes large dosages (200 mg or above) to be injected at a time, as higher

quantities of T can cause more conversion to estradiol.

To my knowledge, there is no recommendation to test for estradiol in the

American Association of Clinical Endocrinologists Guidelines for Hypogonadism,

nor in The Endocrine Society's Guidelines for Hypogonadism, although both

sources indicate 1) that testosteone can convert to DHT or estradiol, and 2)

that higher injection dosages (over 200 mg) are not ideal. But as Phil

mentioned, there are studies showing that lowering estradiol can increase

testosterone (perhaps excluding the need for TRT), & that higher injection

dosages can lead to more aromatization to estradiol.

Often, endos & other docs object to testing for estradiol in men (because they

don't know what to do with the result?), so sometimes it's better to order the

test yourself & then present the results.
