I have fought my insurance company a few times and right

now am doing so for Lyme. I found this site to be very accurate

and helpful and have used the templates in the downloaded

material with some success. I need one third more of my initial

treatment program approved for the first round of treatments

for late disseminated Lyme. Hope this helps you. Don't give up!
