Elva, it is nice of you to get information for your friend, but it is probably

better if you invited your friend to join us and post her questions. As far as

the test results go, an elevated result in isolation might mean little (although

that is quite high). A ratio to other elements tested would be more helpful to

anyone looking to offer advice.

Could she post the entire test? I don't even know what it is; urine? blood?

Is this a Metamatrix test?

Pam,

sorry, but the mom is unable to speak English. She was very despaired as the

doctor in China told her the boy(autistic child) may sudden-death due to the

high l-lactate, however the doctor have no much idea how to deal with it. the

mom doesn't know who she can ask for help in China, so I help post her question

in our group.

thank you very much for your attention! I will attach the boy's whole test

result here.

I hope somebody can offer some advice for you. I, unfortunately, do not

understand enough to comment. I would hope doctors in China would follow up on

these results.

The highly elevated lactate falls under the category of carbohydrate metabolism.

I wonder what inborn errors of metabolism are screened in China at birth?

Perhaps he has an undiagnosed disease/disorder?

I wish I could help more.

A test result like this is almost certainly not real.

If the blood isn't cooled and processed reasonably quickly, the red blood cells

will create a very large amount of lactate.

If it was present in the blood in a living person, there would also be very high

pyruvate.

If there were some genetic disorder where lactate could not be converted to

pyruvate, the fat metabolism markers (adipate, suberate, ethylmalonate) would be

quite elevated.

The pattern of Krebs cycle metabolites also makes no sense.

I would say the test has no real meaning (this particular panel is not useful

for autistic children) and probably it is not practical to get the blood sample

from China to Georgia USA in a timely manner so as to be sure of getting a good

analysis anyway.

A hair test, or discussion of what is up with the kid, might be useful. Chasing

the lactate issue probably is not.

Possibly a group that talks about Mitochondrial dysfunction?  I think I

remember that a high lactate reading indicated mito issues.
