I used the 'religious exemption form' as a template from this link-- http://drtenpenny.com/vac_refusal_forms.aspx in IL state accepts religious beliefs as exempt. Whenever we go to the doctor's and they ask us to write down allergies, we list " Vaccine ingredients. " We do this when we have to take him to the hospital, too. I also write down that he is allergic to mercury and aluminum. Write down he is allergic to formaldehyde and antifreeze. We started to loose him Flu shot only " protects " you from what viruses are in it plus now its combined with the h1n1 in the shot. Deff find out your rights and use them! Then you are not required to do anything. School is not compulsory until age 6. Flu contains aluminum also. Dont have to hv a office visit, just call your dr. to write a note (prescription pad) that your child has egg allergy; therefore he is exempt from flushot.
