For someone 34 lb that would be 4.25 mg. 15 mg for someone 34 lb is about 0.44 mg/lb - almost at the high end of 0.5 mg/lb. If she is doing ok at 15 mg then I would continue with those doses for several months before increasing. I wouldn't round up. From a 15 mg dose one would increase to no more than 22.5. Once you get to a 22.5 dose or so then try 75/3 = 25 mg and if there are too many side effects at that dose try 75/4 = 18.75. The dosing is 1/8-1/2mg per pound of body weight.
