As I read it it sounded like we were living in some sort of " 1984 " twilight zone instead of the USA which is supposed to be a democracy. It is creepy how tyranny pervasively finds its way into a " free " nation. I know when My son was damaged and then my subsequent request for Hg testing resulted in what I can only describe as a Gestapo interrogation. You do know that when the next real pandemic comes rather than blame the immune systems that have been weakened by over-vaccination, the powers that be will blame the unvaccinated. (and the open-minded, and the outside-the-box-thinkers). I will never gain sign any paper a doc asks me to sign without having my lawyer look it over. So don't beat yourself up, just prepare a religious exemption and have that placed in both their files also. You can ask to retract the form permissions as you are replacing it with your religious exemption because you are within your right to refuse vaccines based on religion rather than outright refusing or You can also ask to re-read the form because you were discussing your religious exemption with your lawyer and he suggested you verify that their form was not incriminating. Not all doctors are actually using the form maliciously. It's just that it could be if someone wanted too. I would be sure to present the religious exemption (signed and notarized) for the file. I was legally advised years ago by our informed choice coalition lawyer never to sign those forms, as they are being used by some health departments to try to make you vaccinate. The health departments keep track of who is getting shots and who is not in most states and if a doctor has too few patients receiving vaccines, the DoH starts asking questions. This is why doctors want you to sign a " refusal to vaccinate " form.
