I believe I signed a refusal of medical treatment type of form at our son's annual last month. I'll ask for a copy and look it over more carefully and see what I can do about it, maybe add something to his med file at the doc's? It's probably too late now that it's in his file. I'll have to do the same thing for the refusal of Tdap at my daughter's high school. Great! I can just kick myself for not taking my time to read things more closely. Got any suggestions for me on this? If a doctor offers it to you, tell them you will take it your lawyer before you sign it, take it and don't bring it back. This form is not the same as having a religious objection to vaccination. I was legally advised years ago by our informed choice coalition lawyer never to sign those forms, as they are being used by some health departments to try to make you vaccinate. It is also a legal document that helps the doctor be able to justify why patient X is not vaccinated. The health departments keep track of who is getting shots and who is not in most states and if a doctor has too few patients receiving vaccines, the DoH starts asking questions. This is why doctors want you to sign a " refusal to vaccinate " form.
